Objectives: Azapirone-class drugs are partial 5-HT1A receptor agonists commonly used to treat anxiety disorders. Prior experimental studies have so far demonstrated that these drugs have low potential for dependence and problematic use and are considered safe treatment options compared with benzodiazepines. However, recent evidence suggesting the contrary raises concerns about their safety.

Objective: To evaluate the efficacy of baricitinib in combination therapy for managing refractory, rapidly progressive systemic sclerosis (SSc) with severe cardiac conduction defects and interstitial lung disease (ILD).
Methods: A 48-year-old male patient with SSc complicated by significant cardiac enlargement, third-degree atrioventricular block, heart failure, progressive ILD, and partial intestinal obstruction was included in the study. Prior treatments with mycophenolate mofetil (MMF), tacrolimus, and cyclophosphamide (CTX) had shown limited efficacy.

Transjugular intrahepatic portosystemic shunt (TIPS) placement is a well-established intervention for portal hypertension. However, some patients experience persistent complications such as encephalopathy, ascites, or thrombocytopenia, especially when further TIPS optimization is not technically possible. Partial splenic embolization (PSE), typically performed for hypersplenism or certain hematologic conditions, can reduce portal venous inflow and improve cytopenias; however, its use as an adjunct to TIPS is less well described.

Left-sided partial anomalous pulmonary venous return (PAPVR) may remain clinically silent and undiagnosed until incidentally identified, potentially introducing complexity in perioperative assessment and management, particularly in patients with significant comorbidities. We report the case of a 77-year-old male with metastatic colorectal adenocarcinoma and a history of multiple right-sided pulmonary metastasectomies. He underwent a right completion upper bilobectomy.
