An 80-year-old woman presented to the emergency department with failure to thrive and weakness for 14 days. Medical history was significant for polio. On admission her electrocardiogram showed atrial flutter, and cardiac enzymes were elevated. Echocardiogram revealed a high pulmonary artery pressure, but no other wall motion abnormalities or valvulopathies. Chest x-ray showed a large lucency likely representing a diaphragmatic hernia. Computed tomographic scan confirmed the hernia. Our patient remained in atrial flutter despite rate control, and thereafter surgery was consulted to evaluate the patient. She underwent hernia repair. After surgery, the patient was taken off rate control and monitored for 72 hours; she did not have any episode of atrial flutter and was discharged with follow up in a week showing no arrhythmia. Her flutter was caused directly by the mechanical effect of the large hiatal hernia pressing against her heart, as the flutter resolved after the operation.

Previous studies have shown that a decline in IC is linked to an increased mortality rate. Background: Atrial fibrillation and flutter (AF/AFL) are traditionally regarded as diseases of ageing, with most research focused on elderly populations. In contrast, early-onset AF/AFL (<65 years) is increasingly recognized as a potential marker of cardiomyopathy, yet its global burden remains poorly understood.
Methods: Using data from the Global Burden of Disease 2021 study, we analyzed trends in prevalence, incidence, disability-adjusted life years (DALYs), mortality, and average annual percentage change (AAPC) of AF/AFL among individuals aged 30-64 years from 1990 to 2021.

Aims: To examine the association between elevated body mass index (BMI) and a wide range of vascular and cardiometabolic diseases in men and women.
Materials And Methods: This retrospective cohort study used data from the IQVIA Disease Analyzer database, comprising anonymized records from over 3000 office-based physicians in Germany. We included 233 730 patients aged ≥40 years with at least one recorded BMI measurement between January 2005 and December 2023.
