We report the novel use of the AngioVac device in a percutaneous hybrid approach to remove a large right atrial clot as an effective and potentially lifesaving alternative to a very high-risk redo-sternotomy in a Jehovah's Witness patient.

The link between heart failure (HF) and increased prothrombotic risk has gathered attention, with several studies exploring this association. Patients with HF and severe mitral regurgitation (MR) undergoing transcatheter edge-to-edge repair (TEER) may present enhanced left atrial (LA) function and improve thrombosis-related factors due to the procedure. This study investigates the role of left atrial strain (LAS), assessed via speckle-tracking echocardiography, in detecting subtle LA abnormalities and its potential link to thrombotic risk in severe MR patients.

By examining gene expression of cells comprising stroke clots, we aim to gain valuable insights into the underlying mechanisms of this disease and identify potential biomarkers of stroke cause.
Methods: We employed single-cell RNA sequencing to analyze 10 clot samples from patients diagnosed with large vessel occlusion stroke.

Platelet reactivity (PR) in clopidogrel-treated patients undergoing percutaneous coronary syndrome (PCI) associates with ischemic and bleeding risk. The aim was to investigate the association of PR and global hemostasis with this risk in patients with atrial fibrillation (AF) undergoing PCI. TEG was performed on day 1-3 after PCI.

Background: Scarce data are available on pharmacodynamic (PD) variability in very elderly patients receiving direct oral anticoagulants (DOACs) for atrial fibrillation (AF). Thrombodynamics-4D (TD-4D), which simultaneously assesses fibrin clot formation and thrombin generation, has not yet been tested in patients on rivaroxaban, apixaban, or dabigatran.
Objectives: To (i) evaluate TD-4D's ability to assess DOAC effect added to normal plasma; (ii) assess DOAC PD in very elderly patients with AF along with DOAC concentrations; (iii) identify factors associated with interindividual variability of DOAC PD at peak and trough levels.

The image represents the post-mortem heart of a 28-year-old female patient, diagnosed in childhood with complete common atrioventricular canal defect. At time of diagnosis, the family refused surgery, as did the patient during her adulthood. Despite being advised against pregnancy, she became pregnant.
