Careful consideration of the differential diagnosis of discordant thyroid function tests (TFTs) is fundamental to prevent mismanagement, waste of resources, uncover rare thyroid disorders, or other underlying critical conditions in patients referred for the evaluation of this issue. Here, we describe the frequency of underlying diagnoses and focus on cases in which the identification of analytical interferences led to the discovery of lymphoproliferative disorders. This is a retrospective cross-sectional study including all consecutive patients referred to the Department of Endocrine and Metabolic Diseases, Istituto Auxologico Italiano, for suspected central hyperthyroidism between January 2017 and February 2025.

This brief report describes 2 cases in which paraprotein caused spurious biochemistry results. In both cases standard laboratory measurements of calcium indicated the patients were hypercalcaemic and had significantly raised Vitamin D concentrations in serum. However, use of alternative measurements of calcium and vitamin D showed that these results were spurious.

Objectives: IgM paraprotein-mediated interference in Uric Acid (UA) measurement remains a significant challenge in clinical labs, potentially compromising hyperuricemia diagnosis and management. This study investigated the identification and elimination strategies for IgM interference in the Beckman Coulter AU5800 UA assays.
Methods: Serum samples without hemolysis, icterus, or lipemia from 196 patients with IgM paraproteinemia were analyzed for UA levels based on uricase-peroxidase enzymatic reaction and lipemia (LIP) index levels via transmission turbidimetry.

Objectives: In serum immunofixation electrophoresis, therapeutic monoclonal antibodies (tmAbs) can be misinterpreted as paraproteins. Strategies to circumvent these interferences include the use of anti-tmAb antibodies, which led to the development of the Hydrashift assay. However, this assay is exclusively available for the Sebia platform and limited to specific tmAbs, such as daratumumab (DmAb) and isatuximab (ImAb).
