Artificial Intelligence Use in Daily and Professional Life among Pediatric Surgeons in India: A Roadmap for Adoption Based on Online Survey Results.

Background: Artificial intelligence (AI) is transforming the various domains of medicine, including pediatric surgery, where precision and timely decision-making are critical. However, the real-world integration of AI in pediatric surgery, particularly in low- and middle-income countries such as India, remains underexplored. We propose a roadmap for the adoption of AI based on our survey results for (IAPS).

Three-dimensional Laparoscopic Management of a Large Prostatic Utricle Cyst in a 6-month-old Boy: An Efficient Minimally Invasive Approach.

Prostatic utricle cysts (PUCs) in males with normal external genitalia are rare and often asymptomatic. Minimally invasive surgical techniques, such as laparoscopic excision, have demonstrated effectiveness for symptomatic cases. This report highlights the advantages of a three-dimensional (3D) laparoscopic approach in managing a large PUC in a 6-month-old boy.

Systematic review and meta-analysis on macrovascular and microvascular anatomy of preputial skin in hypospadias and its implications.

Introduction: Hypospadias repair is challenging due to complications like urethrocutaneous fistula and flap necrosis, often linked to preputial vascular anatomy. While macrovascular and microvascular patterns are studied, findings are inconsistent. The impact of preoperative hormonal therapy (PHT) on vascularity and outcomes is unclear.

Umbilical morphometry and centile curves in boys aged 1-14 years: Establishing reference values for umbilical reconstruction.

Unlabelled: Umbilicoplasty is essential for the reconstruction of congenital malformations such as bladder-exstrophy-epispadias complex and gastroschisis. Reference values for umbilical morphometry are critical for anatomically acceptable outcomes.

Objective: To measure vertical height and width of the umbilicus in male participants aged 1-14 years and generate age-specific centile curves for umbilical reconstruction.

Two-flip Mathieu's technique in distal hypospadias with small glans.

Introduction: Hypospadias repair can be complicated, especially with poorly developed glans. This study evaluates the outcomes of the Two-Flip Mathieu (TFM) procedure, which aims to reinforce the glans and reduce dehiscence in patients with small glans, comparing it to conventional Mathieu's repair.

Methods: This retrospective study analyzed case records of patients with distal hypospadias and minimal ventral curvature.

Advancing Precision in Penile Length Measurement: Evidence-based Synthesis of Stretched Penile Length INdicator Technique (SPLINT).

Purpose: The purpose of this study was to synthesize evidence and propose a technique for estimation of stretched penile length (SPL) applicable to children.

Materials And Methods: This review has been conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses. PubMed, Embase, and Scopus databases were queried on penile length (PL) measurement techniques in humans published from 1990 onward.

A Pilot Survey of Indian Stakeholders: Parents, Doctors, and Grown-Up Patients of Disorders of Sexual Differentiation on Management Decisions and Associated Gender Dysphoria.

Background And Aims: Of late, there are many legal representations from select quarters to halt all medical interventions in children with differences of sex development (DSD). In this survey on management decisions in DSD, we distil the views of Indian stakeholders: parents, physicians, and grown-up patients with DSD on their management decisions to identify decisional satisfaction or gender dysphoria.

Methods: The survey domains included the patient demographics, final diagnosis, decision on the sex of rearing, surgical interventions, opinion of the stakeholders on the preferred age of sex assignment, final sex of rearing, and agreement/disagreement about sex assignment (gender dysphoria).

Management, Outcome, Risk, and Expectation Classification for Structural Fetal Anomalies to Aid Antenatal Counseling: A Systematic Review.

The aim of this study was to propose Management, Outcome, Risk, and Expectation (MORE) as a risk based stratification to aid in antenatal parental counseling and decision making through a systematic review of published literature. A Preferred Reporting Items for Systematic Reviews and Meta Analyses compliant systematic review was conducted to include articles that covered antenatal counseling of pediatric surgical conditions. The following information was solicited for each anomaly: primary organ syste*-m of involvement, single or multi system anomaly, natural history of the disease, standard management of the anomaly, need for antenatal intervention, and whether the anomaly requires any alteration in the obstetric management.

Pediatric Idiopathic Chylopericardium: A Case Managed with Thoracoscopic Window Pericardiotomy.

Idiopathic chylopericardium (CP) in the pediatric population is a rare entity with very few reported cases and is characterized by the accumulation of chyle in the pericardial cavity. There are no guidelines for the management of this rare entity. The present study reports a case of idiopathic CP in an infant and our experience of managing it by pericardial window creation using VATS and a multidisciplinary approach providing the optimum care for the child.

Personal financial literacy among Indian postgraduate residents.

Background Postgraduate residents' ability to understand and effectively manage their finances is variable. We conducted this study to assess the awareness of personal financial literacy among Indian residents. Methods We circulated a web-based cross-sectional Google Forms multiple-choice questionnaire having questions in three categories including financial awareness, current financial status and plans among 400 Indian residents.

Laparoscopic-Assisted Modified Posterior Sagittal Anorectoplasty for Rectobulbar Urethral Fistula of Anorectal Malformation: A Prospective Study.

Introduction: Laparoscopic anorectoplasty (LAARP) is useful for the management of rectoprostatic urethral fistula (RPUF), due to easier rectal mobilization, avoidance of posterior sagittal muscular incision, and shorter hospital stay. However, its role in rectobulbar urethral fistula (RBUF) is still debated as there is a chance of urethral diverticulum (UD), due to incomplete dissection. Laparoscopy-assisted modified posterior sagittal anorectoplasty (LAMPSARP) utilizes advantages of LAARP combined with fistula dissection using small sagittal incision preserving puborectalis.

A 10-Year Study of the Outcome of Wilms' Tumor in Central India and Identifying Practice Gaps.

Introduction: Despite remarkable improvement in Wilms' tumor (WT) survival in Western world, sub-optimal outcome in resource-constrained settings is influenced by late presentation, larger size, and poor access to treatment. This prompted us to study the outcome at a tertiary care center and to identify the global and local practice gaps.

Materials And Methods: A retrospective, observational study of WT was conducted from October 2009 to September 2019 at a tertiary care setting.

Surgical innovation in LMICs-The perspective from India.

Surgical Innovations are central to surgical progress, and have led to exponential growth in various fields of Surgery. Surgical Innovations in Lower and Middle Income Countries are the result of creativity of frontline health workers in search of simple, safe and ethical solutions for their unique challenges. The key lies in: 'simplifying the idea/technique/device' to find patients' needs-driven low-cost innovative surgical solutions; which can be used on a wider scale to achieve health equity for underserved populations.

Violence at work is becoming an alarming phenomenon worldwide affecting the millions of health care workers. This study was conducted to assess the violence towards Resident doctors in Indian teaching hospitals. Google forms questionnaire was developed and circulated electronically to resident doctors working in India.

Double Rectangle Fascia Lata Frontalis Sling: A Rationale Approach for Ptosis.

 The purpose of this study was to evaluate the functional outcomes of a modified technique of double rectangle pattern for correction of severe ptosis.  This is a retrospective study over a period of 8 years including patients who underwent correction of ptosis by double rectangle using autologous fascia lata sling. Surgical outcomes were assessed postoperatively by distance from the corneal light reflex to the upper eyelid margin (MRD1) and levator function.

Nosocomial SARS-CoV-2 transmission in postoperative infection and mortality assumes great importance given the gradual re-booting of elective surgeries all over the world. It is interesting to see that laparoscopic surgery may be protective for nosocomial transmission of COVID-19 infection owing to reduced duration of hospital stay. For the same reason, the wider application of enhanced recovery after surgery protocols need to be explored.

Strategies for Optimizing the Use of PPE During Surgery in COVID-19 Pandemic: Rapid Scoping Review of Guidelines.

Personal protective equipment (PPE) plays a fundamental role in the prevention of spread to Health Care Professionals (HCP) ; especially in a surgical setting. This scoping review of surgery guidelines was performed to appraise the quality of appropriate PPE recommendations and propose a strategy to optimize the PPE usage. This rapid scoping review of guidelines on surgery during COVID-19 was conducted according to Preferred Reporting Items for Systematic reviews and Meta-Analyses extension for Scoping Reviews protocol.

Etiology-Based Decision-Making Protocol for Pediatric Cholelithiasis.

Objectives: We reviewed hospital records of pediatric cholelithiasis to develop an etiology-based decision-making protocol.

Methods: This retrospective study was conducted on consecutive pediatric cholelithiasis patients from July, 2014 to June, 2019 in a tertiary care center. Pediatric cholelithiasis was classified according to etiology, and the outcome of medical/surgical treatment was noted.

Impact of "Lateralizing" the Sigmoid Colostomy on Port Ergonomics in Laparoscopic Anorectoplasty for High Anorectal Malformations.

Introduction:  The high-type anorectal malformations (ARM) are conventionally managed by an initial left iliac fossa sigmoid colostomy, followed by laparoscopic anorectoplasty (LARP). Such a stoma occupies left half of the infraumbilical region and hinders the LARP ergonomics, leading to the surgeon's discomfort. We studied the outcome and impact of "lateralizing" (shifting laterally in the abdominal wall) the colostomy on port ergonomics.

Comparison of 3D vs 2D laparoscopic-assisted anorectal pull-through (LAARP) for high anorectal malformations in children.

Introduction: The limitation of two-dimensional (2D) laparoscopic techniques includes lack of stereoscopic vision and depth perception which can affect surgical performance, physical and mental comfort of the operating surgeon. 3D laparoscopic surgery is popular in adults; however, its application and experience in the pediatric age group have been limited. We did a comparison of 2D and 3D laparoscopic-assisted anorectal pull-through (LAARP) in male high anorectal malformations (ARM).
