Purpose: To compare the utility of two large language models (LLM) in dry eye disease (DED) clinics and research.

Methods: Trained ocular surface experts generated 12 prompts for queries commonly asked by DED patients ( = 10 questions) and research ideas for DED ( = 2). Responses of two LLM models, ChatGPT4 and Perplexity AI, were graded by them using a standardized grading system (1 = needs improvement, 2 = fair, 3 = good, and 4 = excellent) evaluating the response accuracy, compassion, comprehensiveness, professionalism, humanness, and overall quality of each response.

This report provides an evidence-based review of current strategies to manage dry eye disease (DED). First-line management focuses on methods to replenish, conserve and stimulate the tear film, with an emphasis on ocular supplements, which remain the cornerstone of DED treatment. Meibomian gland dysfunction, a primary contributor to DED, is typically treated with warm compresses and a wide variety of in-office treatments, including device-driven technologies to warm the eyelids, intense pulsed light therapy, low-level light therapy and other new and emerging technologies.

Proliferative Vitreoretinopathy (PVR) is the most common cause of surgical failure after retinal detachment (RD) repair, complicating up to 10% of spontaneous RD repairs and 50% of open globe injury-related RD. Early surgical intervention is currently the only intervention that reduces PVR incidence. An effective non-surgical intervention would be valuable in reducing PVR incidence and/or severity, particularly where access to surgery is limited or may be delayed.

Corneal neuropathic pain (CNP) is a poorly defined disease entity characterised by an aberrant pain response to normally non-painful stimuli and categorised into having peripheral and central mechanisms, with the former responding to instillation of topical anaesthetic. CNP is a challenging condition to diagnose due to numerous aetiologies, an absence of clinical signs and ancillary tests (in vivo confocal microscopy and esthesiometry), lacking the ability to confirm the diagnosis and having limited availability.
