The objective of this study was to examine the differences in clinical presentation and complications during the first year of follow-up in patients who experienced an episode of acute diverticulitis (AD) with pericolic free gas, comparing outcomes across different age groups. A prospective subanalysis was conducted on a cohort from the ADiFAS study, which originally included 1099 patients. After excluding 289 patients who did not meet the inclusion criteria, 810 patients remained for analysis.

Background: The detection of indeterminate pulmonary nodules (IPN) at diagnosis of colorectal cancer (CRC) has increased. However, there is limited information on predictive factors for its progression (pPF) to pulmonary metastases (PM). This study aims to identify these pPF to select appropriate management strategies.

Background: Structured training programs for robotic colorectal surgery are limited, and there are concerns about surgical outcomes and operating times.

Objective: To compare perioperative and oncological outcomes of robotic total mesorectal excision for rectal cancer performed by expert consultants and surgical trainees in a modular surgical training program.

Design: Retrospective cohort study.

Background: As the population ages, more older adults are presenting for surgery. Age-related declines in physiological reserve and functional capacity can result in frailty and poor outcomes after surgery. Hence, optimizing perioperative care in older patients is imperative.

Article Synopsis
  • Colon cancer is becoming more common in younger people, and treatment usually involves surgery and chemotherapy; however, neoadjuvant chemotherapy is being tested for its potential to improve survival rates.
  • This study is a randomized clinical trial that will compare two treatment groups: one receiving neoadjuvant chemotherapy, surgery, and adjuvant chemotherapy, and the other getting surgery followed by adjuvant chemotherapy.
  • The main goal is to see if neoadjuvant chemotherapy leads to better survival outcomes within two years, while also looking at other factors like tumor response and side effects.

Article Synopsis
  • The study aimed to compare the effectiveness of transanal versus laparoscopic Total Mesorectal Excision in enabling sphincter-saving interventions for obese rectal cancer patients.
  • Out of 93 patients, the transanal group showed a significantly higher rate of sphincter-saving surgeries (97%) compared to the laparoscopic group (71%).
  • While both approaches had similar major complication rates, the transanal approach had no conversions to open surgery, suggesting it may offer safer outcomes in complex cases.

Purpose: Rectal cancer surgery presents challenges in achieving good oncological results and preserving functional outcomes. Different surgical approaches, including open, laparoscopic, robotic and transanal techniques, have been employed, but there is a lack of consensus on the optimal approach, particularly in terms of functional results. This study aims to assess bowel function and to compare outcomes of patients that had undergone surgery for mid-low rectal cancer across different surgical approaches.

Background: There are no specific recommendations regarding the optimal management of this group of patients. The World Society of Emergency Surgery suggested a nonoperative strategy with antibiotic therapy, but this was a weak recommendation. This study aims to identify the optimal management of patients with acute diverticulitis (AD) presenting with pericolic free air with or without pericolic fluid.

For T4 rectal tumours and local recurrences (LR) of rectal cancer, a radical resection beyond TME, sometimes by multi-visceral resection, is important to obtain safe margins and improve survival. The use of the laparoscopic approach (LA) for these cases is still controversial and associated with a high rate of conversion. However, robotic surgery might offer some advantages that can overcome some of the limitations of LA.
