Case: A 6-year-old boy presented with a low-grade fibromyxoid sarcoma located at the base of the thumb following an unplanned resection. A wide resection was performed, including removal of the proximal first metacarpal, trapezium, thenar muscles, and thumb tendons. Reconstruction was performed using a parascapular osteocutaneous flap to achieve stable thumb opposition position, combined with free tendon grafts to restore thumb interphalangeal and metacarpophalangeal joint motion.

Methods: This study included 56 patients with spinal metastases who had Spinal Instability Neoplastic Scores (SINS) of 7-12 and who underwent radiotherapy and intensive rehabilitation without a bed rest period as initial treatment between 2020 and 2023. We evaluated progression of paralysis and changes in spinal alignment before treatment, within 1 month, 1 to 6 months, and more than 6 months after treatment.

The skin defect was reconstructed with free latissimus dorsi flap, ER with tensor fascia lata, and ETs with 2-staged tendon reconstruction using silicone rod interposition, followed by semitendinosus and gracilis tendon grafts. Four years postoperatively, she achieved 92% of ankle and 70% of great toe motion of the contralateral side.
