Background: Weight gain is common in treatment naïve people with HIV (PWH) initiating antiretroviral therapy (ART). The mechanisms driving this weight gain are unclear. The current study tested the hypothesis that bone-derived hormones are associated with weight gain with ART initiation and that the associations are antiretroviral (ARV) specific.

Importance: Sexually transmitted infections (STIs) pose a substantial public health challenge, but fragmented reporting systems have limited capacity for disease surveillance approaches that might support a learning public health system, where near real-time data-driven insights are integrated to inform, evaluate, and adapt public health strategies.

Objective: To examine how health information exchanges and open government data can be leveraged to foster a learning public health system by characterizing and comparing patterns in diagnosis vs testing at a scale commensurate with regional public health agencies.

Design, Setting, And Participants: In this cross-sectional study, chlamydia, gonorrhea, and HIV testing and diagnosis between January 1, 2018, and June 30, 2023, were assessed to study concurrent testing and coinfections and estimate correlates of laboratory testing and positive results.

Polymyxins are used against highly resistant Gram-negative pathogens, and nephrotoxicity (acute kidney injury, AKI) is the major dose-limiting adverse effect. We aimed to develop a well-informed population pharmacokinetic/toxicodynamic (PK/TD) model using the largest polymyxin B-treated critically ill patient population studied to inform polymyxin B dosing strategies. Critically ill patients receiving intravenous polymyxin B for at least 48 hours were enrolled in an observational study.

Purpose Of Review: Osteoporosis and fragility fractures continue to be a concern for aging people living with HIV (PLWH), despite newer antiretroviral (ART) formulations that are associated with reduced bone toxicity. The aim of this review is to evaluate recent literature focusing on estimates of osteoporosis and fractures in various study populations, efficacy of current fracture risk assessment tools, and interventions to improve bone health outcomes.

Recent Findings: Prevalence of low bone mineral density (BMD) remains higher among PLWH globally, with new estimates ranging from 24 to 59%.

The persistence of latent HIV-1 proviruses in CD4 T cells is a major obstacle to curing HIV. The "shock and kill" strategy involves reversing latency with latency-reversing agents (LRAs) and selectively inducing cell death in infected cells. However, current LRAs have shown limited efficacy in eliminating the ex vivo HIV reservoir and thus failed in clinical study.

Background: Several commercial programs incorporate artificial intelligence in diagnosis, but very few dental professionals have been surveyed regarding its acceptability and usability. Furthermore, few have explored how these advances might be incorporated into routine practice.

Methods: Our team developed and implemented a deep learning (DL) model employing semantic segmentation neural networks and object detection networks to precisely identify alveolar bone crestal levels (ABCLs) and cemento-enamel junctions (CEJs) to measure change in alveolar crestal height (ACH).

Ischemic heart disease and acute myocardial infarction (AMI) is a leading cause of morbidity and mortality. Improvements have been made in coronary interventions to restore blood flow, but ischemia/reperfusion (I/R) injury significantly impacts clinical outcomes. We previously reported that activation of percutaneous mechanical unloading of the left ventricle (LV) with a transvalvular axial-flow device simultaneously with reperfusion improves myocardial salvage.

Although emerging data have revealed the critical role of memory CD8 T cells in preventing and controlling SARS-CoV-2 infection, virus-specific CD8 T-cell responses against SARS-CoV-2 and its memory and innate-like subsets in unvaccinated COVID-19 patients with various disease manifestations in an HLA-restricted fashion remain to be understood. Here, we show the strong association of protective cellular immunity with mild COVID-19 and unique cell types against SARS-CoV-2 virus in an HLA-A2 restricted manner. ELISpot assays reveal that SARS-CoV-2-specific CD8 T-cell responses in mild COVID-19 patients are significantly higher than in severe patients, whereas neutralizing antibody responses against SARS-CoV-2 virus significantly correlate with disease severity.

Background: Injectable depot medroxyprogesterone acetate (DMPA) is the most common contraceptive choice among young women in Uganda, where HIV burden is high and HIV preexposure prophylaxis (PrEP) may be offered. For young women who choose to use both agents concurrently, it is unknown whether they will experience declines in bone mineral density (BMD) beyond those elicited by either product singly.

Methods: From 2018 to 2022, we conducted a 2-year prospective study with women ages 16-25 years in Kampala, Uganda desiring pregnancy and HIV prevention.

Article Synopsis
  • Men with HIV show increased alveolar bone loss compared to HIV-negative men, suggesting a link between HIV infection and periodontal issues.
  • The study involved 93 men, aged 35-70, who underwent periodontal exams and analysis of gingival crevicular fluid and X-rays to assess periodontal health.
  • Findings indicate that IL6 cytokine levels in men with HIV may contribute to this greater bone loss, highlighting the need for better oral health care for people living with HIV and further investigation into this area.

Article Synopsis
  • - The study aimed to assess the effectiveness of a new point-of-care urine tenofovir (POC TFV) test in improving the accuracy of self-reported adherence to preexposure prophylaxis (PrEP) among sexually active HIV-negative women in Uganda.
  • - Over 24 months, participants who took the test reported higher rates of low PrEP adherence and condomless sex during visits with the test compared to those without it, indicating that the test may encourage more honest self-reporting.
  • - The accuracy of self-reported PrEP adherence significantly increased when the POC TFV test was utilized, suggesting that the test could foster better communication between patients and healthcare providers.

The field of orthodontics has seen a recent increase in the number of patients over the age of 50 seeking treatment and also an increase in the use of cone beam technology. Similar to other joints in the body, the temporomandibular joint (TMJ) is associated with age-related degeneration. However, unlike other joints, degeneration of the TMJ is rarely symptomatic and when there is pain, it is usually self-limiting.

Background: Periodontal health in men with HIV remains understudied, despite suggestions of associations between HIV infection and gingival pocketing, periodontal attachment loss, and gingival inflammation. As antiretroviral therapy (ART) has improved the quality of life for people living with HIV (PLWH), aging-related risk factors and comorbidities, including periodontitis, have emerged. This study aims to assess alveolar bone height, gingival crevicular fluid (GCF) cytokines, and periodontal disease activity in men with and without HIV.

Article Synopsis
  • The HeartMate 3 (HM3) is the only fully magnetically levitated left ventricular assist device (LVAD) available for advanced heart failure patients, but its impact on heart function post-implantation hasn't been fully explored.
  • A study comparing HM3 to older LVADs (HeartWare Ventricular Assist Device and HeartMate II) found no significant differences in heart function improvements after implantation.
  • The proportion of patients showing significant reverse heart remodeling was similar across all devices, indicating that HM3 technology may be just as effective as older models in supporting heart recovery.
