The concept of agnostic biomarkers-molecular modifications that guide therapy irrespective of tumor origin-has gained increasing relevance in oncology, including colorectal cancer (CRC). This review aims to critically evaluate the role of such biomarkers in CRC, highlighting their clinical significance as therapeutic targets and indicators of prognosis. Through a PubMed search using the terms "agnostic treatment AND colorectal cancer," eight key studies were identified and qualitatively analyzed.

Crohn's disease (CD) is a chronic inflammatory disorder of the gastrointestinal tract that often leads to intestinal fibrosis, an irreversible complication associated with strictures and the need for surgical intervention. Fibrosis occurs due to prolonged inflammation and abnormal wound healing, involving complex interactions between immune cells, mesenchymal cells, cytokines, and the gut microbiota. Key fibrogenic mechanisms include the activation of fibroblasts and myofibroblasts, cytokine signaling, and disrupted turnover of the extracellular matrix.

Perioperative and long-term postoperative major adverse cardiovascular events (MACE) are a leading cause of morbidity and mortality in patients undergoing noncardiac surgery. In selected high-risk patients, when information about cardiovascular status may influence surgical decisions, preoperative risk stratification is reasonable, with stress imaging being the preferred method. Coronary computed angiography (CCTA) and coronary artery calcium score (CACS) offer direct anatomical assessment of atherosclerotic coronary arteries and help gauge the extent and severity of coronary artery disease.

Article Synopsis
  • Managing complex perianal fistulizing Crohn's disease is tough, but darvadstrocel, which uses fat-derived stem cells, shows promise in promoting healing and tissue regeneration.
  • A literature review identified several studies, including randomized controlled trials, that found darvadstrocel is more effective than a placebo in improving symptoms over time, with similar rates of side effects between the treatment and control groups.
  • While darvadstrocel seems to be a safe and effective option for treating complex perianal fistulas, further randomized clinical trials are necessary to confirm its efficacy and safety.
