Over the last 2 decades, the incidence of prosthetic shoulder replacements has increased considerably, leading to a rising number of complications and reoperations. In this article, we review how SPECT/CT can be of significant clinical value for shoulder surgeons. Bone SPECT/CT can provide the correct diagnosis for different types of mechanical complications: glenoiditis after hemiarthroplasty, glenoidal loosening after total shoulder arthroplasty, and scapular notching after reverse shoulder arthroplasty.

Postoperative pain is a clinically relevant issue in orthopedic patients, affecting more than 40% 1 year after foot and ankle surgery. Because of the very complex anatomy with many different joints and several motion axes, clinical examination and conventional imaging are sometimes not sufficient to identify a local pain generator. Local uptake of bone-seeking radiopharmaceuticals is known to correlate accurately with sites of pain generating foci and, thus, bone scintigraphy has been an established method to evaluate these respective patients for many years.

The medical history of the patient revealed an opening wedge high tibial osteotomy performed nine years earlier, with insertion of two blocks of ceramic made of hydroxyapatite and tricalcium phosphate in a wedge configuration as synthetic bone substitutes. The porous structure of these implants is analogous to the architecture of cancellous bone and permits fibrovascular and bone ingrowth, promoting the healing process.
