Purpose: Imaging is a cornerstone of the osteoarthritis (OA)-arthroplasty clinical pathway: the continuum of care that patients with OA undergo, from initial diagnosis through to arthroplasty and postoperative follow-up. With growing interest from the orthopaedic community, this meta-analysis broadly evaluates the performance of deep learning algorithms in interpreting radiographs and cross-sectional imaging in this pathway. The authors hypothesise that deep learning algorithms will have comparable performance to clinicians when interpreting radiographs, but not cross-sectional imaging, with negligible difference in diagnostic and prognostic tasks.

Background: Individuals with inflammatory bowel disease (IBD) often experience pain, mood disturbances, and sleep disruption, which may lead to greater use of pain-relieving and sedative medications compared with the general population. These are associated with increased mortality, paradoxical worsening of pain, and inappropriate IBD treatment discontinuation. Chronic prescribing and co-prescribing increase the risk of respiratory depression, dependence, and overdose.

The emergence of large language models (LLMs) opens new horizons to leverage, often unused, information in clinical text. Our study aims to capitalise on this new potential. Specifically, we examine the utility of text embeddings generated by LLMs in predicting postoperative acute kidney injury (AKI) in paediatric cardiopulmonary bypass (CPB) patients using electronic health record (EHR) text, and propose methods for explaining their output.

Background: Psychotic disorders are severe mental health conditions frequently associated with long-term disability, reduced quality of life and premature mortality. Early Intervention in Psychosis (EIP) services aim to provide timely, comprehensive packages of care for people with psychotic disorders. However, it is not clear which components of EIP services contribute most to the improved outcomes they achieve.

Article Synopsis
  • - The study focuses on understanding the diagnosis process and management of Chronic Obstructive Pulmonary Disease (COPD) in the UK, where nearly 400 million people are affected globally, making it a significant health issue.
  • - Researchers tracked patient data over three time periods to analyze how long it takes to get diagnosed with COPD after symptoms appear, as well as the frequency of acute exacerbations—a period when symptoms worsen significantly.
  • - The results indicated that most diagnoses happen in primary care and revealed variations in how different practices comply with established guidelines; the study highlights the need for improved adherence to these guidelines in COPD management.

Background: Few studies have quantified multimorbidity and frailty trends within hospital settings, with even fewer reporting how much is attributable to the ageing population and individual patient factors. Studies to date have tended to focus on people over 65, rarely capturing older people or stratifying findings by planned and unplanned activity. As the UK's national health service (NHS) backlog worsens, and debates about productivity dominate, it is essential to understand these hospital trends so health services can meet them.
