We administered cryoballoon ablation to a 61-year-old female patient for recurrent paroxysmal atrial fibrillation. Contrast-enhanced CT angiography used to assess the patient's pulmonary vein anatomy prior to cryoballoon ablation revealed an accessory pulmonary vein emanating from the left atrial roof. Cryoballoon ablation was successfully performed on this accessory vein, which is rarely seen in the literature for the first time.

Pulmonary vein (PV) isolation by catheter ablation is a widely used curative therapy for atrial fibrillation (AF). However, in patients with persistent AF (PeAF), long-term outcomes are poor when PV isolation is performed alone. Although left atrial (LA) roof ablation is sometimes added to PV isolation, its effectiveness and predictors of success remain unclear.

Background: Single-shot ablation systems are widely used for pulmonary vein isolation (PVI) in atrial fibrillation (AF). The use of novel pulsed-field ablation (PFA) systems enables ablation beyond PVI, such as left atrial roof ablation (LARA), which may improve outcomes in persistent AF.
Objective: This study aimed to compare the acute efficacy, feasibility, and safety of PVI combined with LARA using three different single-shot ablation systems in patients with persistent AF and left atrial enlargement undergoing their first AF ablation.

Background: The novel POLARx cryoballoon ablation (CBA) system offers enhanced cooling properties for the treatment of atrial fibrillation (AF); however, these capabilities may damage adjacent tissues. This study aimed to assess the prevalence and predictors of gastric hypomotility (GH) in patients undergoing pulmonary vein isolation (PVI) and left atrial (LA) roofline ablation using the POLARx.
Methods: Patients who underwent CBA for PVI using the POLARx system were included in this study.
