Primary gastric squamous cell carcinoma (GSCC) is rare, typically associated with poor survival rates and unsatisfactory outcomes from conventional treatments including surgery, radiotherapy, and chemotherapy. It remains unclear whether nivolumab is as effective for GSCC as it is for gastric adenocarcinoma. Herein, we present the case of a 66-year-old man diagnosed with Epstein-Barr virus-positive metastatic GSCC, involving the liver, multiple lymph nodes, and invasion into the spleen, pancreatic body and tail, and splenic vein. The patient received nivolumab with oxaliplatin, leucovorin, and fluorouracil plus local radiotherapy as first-line treatment; nivolumab, paclitaxel polymer micelles, and carboplatin as the second-line treatment, and nivolumab and apatinib as the third-line treatment. The patient responded remarkably (survival period for 49 months) with manageable treatment toxicity. Genomic and immune characteristics were analyzed to understand the underlying disease mechanisms. In conclusion, integrated therapy (immunotherapy, chemotherapy, local radiotherapy, and anti-angiogenic therapy) may be the ideal treatment approach for patients with GSCC with multiple metastases; however, prospective studies are needed to verify its efficacy.

The tumor microenvironment (TME) of chronic inflammation-associated cancers (CIACs) is shaped by cycles of injury and maladaptive repair, yet the principles organizing fibrotic stroma in these tumors remain unclear. Here, we applied the concept of hot versus cold fibrosis, originally credentialed in non-cancerous fibrosis of heart and kidney, to lung squamous cell carcinoma (LUSC), a prototypical CIAC. Single-cell transcriptomics of matched tumor and adjacent-normal tissue from 16 treatment-naive LUSC patients identified a cold fibrotic architecture in the LUSC TME: cancer-associated fibroblasts (CAFs) expanded and adopted myofibroblast and stress-response states, while macrophages were depleted.

Background: Standard adjuvant chemoradiotherapy (60-66 Gy) following surgery for HPV-associated oropharyngeal squamous cell carcinoma has excellent oncological control but high treatment morbidity. We aimed to compare toxicity of a 30-36 Gy regimen of de-escalated adjuvant radiotherapy and standard of care treatment.
Methods: We did this phase 3, open-label, randomised controlled trial in two academic sites in the USA.

Two new 14,18-cycloapoeuphane triterpenoids, cumingianols G (1) and H (2), along with 12 known compounds, cumingianol C (3), cumingianol A (4), cumingianol D (5), 3,3-ethylenedioxy-5α-cycloart-24-en-23-one (6), 24,25(,)-24,25-epoxy-20()-hydroxydammar-3-one (7), (3β,7α)-stigmast-5-ene-3,7-diol (8), 7α-hydroxystigmasterol (9), 7β-hydroxysitosterol (10), 7β-hydroxystigmasterol (11), ethylcholest-5-en-3-hydroxy-7-one (12), coniferaldehyde (13) and 4-hydroxy-3,5-dimethoxy-benzaldehyde (14), have been isolated from collected in Taiwan. The structures of these metabolites were determined through mass spectrometry and 1D and 2D NMR analyses, combined with comparisons to reference data. The cytotoxic effects of these isolates were evaluated against human oral squamous cell carcinoma (SCC2095), human breast adenocarcinoma (MCF-7), and human gastric adenocarcinoma (SCM-1).

A 68-year-old man came to evaluate a gastric polyp discovered during a routine gastroscopy. After endoscopic mucosal resection, pathological findings confirmed that it's a hyperplastic polyp. Notably, squamous metaplasia was observed within the hyperplastic polyp, and both squamous and glandular epithelium exhibited high-grade intraepithelial neoplasia.
