Article Abstract

Background: With advances in technology, temporary mechanical circulatory support (tMCS) devices have increasingly been used to treat patients in cardiogenic shock.

Case Summary: A 25-year-old G1P0 woman with known idiopathic dilated cardiomyopathy since infancy was hospitalized with acute on chronic decompensated heart failure (HF) at 30 weeks of gestation and bridged to delivery and heart transplantation with an Impella CP heart pump.

Discussion: There is a dearth of experience in use of tMCS for patients experiencing peripartum HF reported in the literature and significant uncertainty over the use of anticoagulation antepartum. This case addresses both antepartum heart pump use and a safe anticoagulation strategy with tMCS in pregnancy.

Take-home Messages: This case is the first reported experience to our knowledge that antepartum Impella heart pump placement with anticoagulation is safe and effective for supporting both mother and fetus to delivery and subsequent heart transplantation for the mother in pregnancy-associated acute decompensated HF. Both mother and baby in this case had successful outcomes.

Ventricular tachycardia disrupts the heart's coordinated pump function, leading to sudden cardiac death. Neutrophils, which are recruited in high numbers to the ischemic myocardium, promote these arrhythmias. Comparing neutrophils with macrophages, we found that resistin-like molecule γ ( or RELMγ) was the most differentially expressed gene in mouse infarcts.

Objectives: Early prediction is crucial for cardiac surgery-associated acute kidney injury (CSA-AKI). We aimed to develop and validate a simple, clinical and laboratory-based risk score system for better CSA-AKI prediction.

Methods: We developed a new pre-operative risk score system for moderate-to-severe CSA-AKI in a 10-year cohort of patients undergoing coronary artery bypass grafting at one tertiary center.

Pathophysiologic assumptions and clinical data frame a promising role for intra-aortic balloon pump (IABP) in heart failure-related cardiogenic shock (HF-CS). Currently, two randomized clinical trials (RCT) exploring this hypothesis have been published, yielding inconclusive results. We thus designed this meta-analysis to assess the efficacy of IABP in HF-CS.

Purpose: In patients with non-ST-segment elevation acute coronary syndrome (NSTEACS), coronary pathology ranges from normal vessels to severe obstructive disease. In NSTEACS patients where invasive coronary angiography (ICA) is recommended, it is unknown whether a non-invasive coronary computed tomography angiography (CCTA) may be used for patient triage. The Very Early Computerized Tomography to Organize Revascularization in patients with NSTEACS (VECTOR) study was a pilot study that assessed whether CCTA can be used to safely discharge NSTEACS patients without obstructive coronary artery disease.
