Objectives: Previous studies have demonstrated the advantages of mitral valve repair (MVP) in infective endocarditis (IE) in early and late outcomes. However, the influence of complexity in mitral valve surgery on long-term outcome is unclear. The purpose of this study was to clarify whether the severity of IE affects long-term outcome.
Methodology: Fifty-one patients who underwent mitral valve surgery during the active phase of native IE were retrospectively reviewed. Severity score was used as an index for the severity of IE. Severity score assigns a score derived from two aspects: (1) the extent of valvular destruction and (2) technical difficulties for predicting the feasibility of MVP reported by our previous studies. Patient profile, long-term survival, and freedom from recurrence of mitral regurgitation (MR) were compared between those with a severity score of 7 or less (: = 17) and with a severity score of 8 or more (: = 34). Recurrence of MR was defined as a more than moderate grade of MR.
Results: Body weight was lower and body surface area was smaller in , and concomitant aortic valve operation was more frequently performed in than in . MVP was more frequently performed in than in (88% vs. 38%, < 0.001), and patients in showed a better 10-year survival rate than those in (100% vs. 70%; log-rank, = 0.020). Freedom from recurrence of MR in 10 years after MVP was 100% vs. 53% in and , respectively (= 0.008).
Conclusions: Severity of IE is associated with long-term outcome. Patients with low severity have high feasibility of MVP, low risks of reoperation, and good long-term survival. In patients with high severity, the indication for complex repair should be considered, taking age and future redo surgery into consideration.

Secondary mitral regurgitation (SMR) is common in patients with heart failure (HF). Although randomized clinical trials have been focused on the treatment of severe SMR, the prognostic role and potential for treatment of moderate SMR cannot be overlooked. The randomized RESHAPE-HF2 trial included patients with moderate and severe SMR with consistent findings in both groups, raising the hypothesis that transcatheter correction of moderate SMR could have beneficial effects, although this needs further investigation.

Background: Routine preoperative echocardiograms (ECHOs) are frequently obtained in patients undergoing minimally invasive repair of pectus excavatum (MIRPE), but the benefit and necessity of preoperative screening remain debated. Objective: Societal guidelines recommend vitamin K antagonists (VKAs) for atrial fibrillation patients with recent biological valve implantation, but the safety and efficacy of direct oral anticoagulants (DOACs) in this setting remain uncertain, especially in the early postoperative period. This substudy of the Left Atrial Appendage Occlusion Study (LAAOS) III trial aimed to compare thromboembolic and bleeding outcomes in patients discharged on VKAs versus DOACs after bioprosthesis implantation or mitral valve repair.
Methods: A total of 2,645 patients were included, with 461 discharged on DOACs and 2184 on VKAs.
