This is the first reported case demonstrating endoscopic ultrasound (EUS) findings in type Va duplication. The malformation of the congenital extrahepatic bile duct is classified as type Va and is rarely observed. To date, not more than 30 cases have been reported in Western literature. For the medical literature, a 50-year-old man with a type Va extrahepatic bile duct obstruction is examined. He presented with abdominal pain and jaundice, and imaging showed biliary dilatation and cholelithiasis. During the EUS procedure, an endoscopic examination revealed the presence of a duplicated common bile duct with a septum. Endoscopic retrograde cholangiopancreatography of the biliary tract also showed the same anomaly, with the presence of obstructive biliary sludge. The case highlights the need to consider EUS as a potent alternative diagnostic method. It is important to detect these anomalies immediately to reduce the chances of endoscopic/surgical complications.

An 81-year-old man was treated with prednisolone, avacopan, and rituximab for microscopic polyangiitis and sulfamethoxazole/trimethoprim (SMX/TMP) and vonoprazan for prophylaxis. The liver enzyme levels were elevated 42 days after avacopan administration. Avacopan, SMX/TMP, and vonoprazan treatment were discontinued.

During this period, a 22-mm cystic mass was identified in the pancreatic head. Its size gradually increased, and she was eventually referred to our department.

Background: Early diagnosis of cholangiocarcinoma (CCA) remains challenging, but liquid biopsy is emerging as a promising detection strategy. Here, we identified a novel bile biomarker for CCA and developed an optic fiber biosensor integrated with digestive endoscopy for real-time diagnosis in vivo.
Methods: A total of 583 subjects and two proteomic analyses were used to screen and validate biomarkers for CCA, and then the corresponding antibodies were generated to construct a surface plasmon resonance (SPR)-based optic fiber biosensor.
