Rationale: Obstructive sleep apnea (OSA) may impair perivascular diffusivity, a proposed glymphatic biomarker, contributing to cognitive decline.
Objectives: To investigate the longitudinal impact of OSA on cognitive performance and perivascular diffusivity, measured by diffusion tensor imaging along the perivascular space (DTI-ALPS), and whether changes in diffusivity mediate the relationship between OSA and cognition.
Methods: In a community-based cohort, 1,110 participants underwent polysomnography, brain DTI, and cognitive assessments at baseline and four-year follow-up. Participants were categorized by apnea-hypopnea index (AHI) at baseline and further grouped longitudinally as OSA-free, improved OSA, stable OSA, or worsened OSA. DTI-ALPS was calculated at both time points to assess perivascular diffusivity. Multivariable models were adjusted for age, sex, education, body mass index, smoking, alcohol use, hypertension, diabetes, total sleep time, and sleep efficiency. Longitudinal models (Δ-values) also included baseline outcome values.
Measurements And Main Results: Higher AHI was consistently associated with lower DTI-ALPS index. Each 1-event/h increase in AHI corresponded to a 0.008 decrease at baseline and a 0.007 decline over four years (both <0.001), with the worsened OSA group showing the greatest reduction. Increases in DTI-ALPS were associated with improvements in visual memory, including immediate recall (beta=0.401), delayed recall (beta=0.351), and recognition (beta=0.145; all <0.001). ∆DTI-ALPS mediated the relationship between ∆AHI and changes in immediate visual recall (beta=-0.015), delayed recall (beta=-0.021), and recognition (beta=-0.016; all <0.001).
Conclusions: OSA severity was associated with altered perivascular diffusivity and related changes in memory, suggesting a potential pathway linking OSA to cognitive impairment via perivascular fluid dynamics.

Although intracerebral hemorrhage (ICH) and cerebral small vessel disease (cSVD) have long been considered distinct clinical entities, emerging evidence reveals significant overlap in their etiologies and imaging markers. This review aims to explore the relationship between ICH and cSVD, suggesting that ICH may represent an acute manifestation of small vessel disease. ICH is primarily caused by cerebral amyloid angiopathy and hypertension, while cSVD is mainly attributed to cerebral amyloid angiopathy and arteriolosclerosis.

Methods: The authors utilized data from the publicly available University of California San Francisco Preoperative Diffuse Glioma MRI (UCSF-PDGM) dataset, which includes 501 adult patients with histopathologically confirmed diffuse glioma, per the 2021 WHO classification, who underwent preoperative MRI, initial tumor resection, and tumor genetic testing at a single medical center from 2015 to 2021.The ALPS index was calculated from diffusivity maps for noninvasive glymphatic system (GS) analysis.

Purpose: Diffusion-weighted imaging (DWI) and intravoxel incoherent motion (IVIM) imaging are well-established approaches for evaluating cerebrospinal fluid (CSF) flow in subarachnoid and perivascular spaces, and have recently been applied to study ventricular CSF flow. However, DWI does not directly measure flow velocity, and the physical implications of DWI measurements are unclear. This study aimed to provide a theoretical interpretation of the DWI and IVIM imaging of CSF flow velocity fields.
