Background: Upper gastrointestinal bleeding (UGIB) can lead to significant morbidity and mortality. Assessing the need for endoscopic interventions and predicting in-hospital mortality are critical, particularly where access to endoscopy is limited.
Objective: This study evaluates the predictive accuracy of pre-endoscopy risk-scoring systems for identifying the need for endoscopic interventions and assessing in-hospital mortality in UGIB patients.
Methods: We conducted a retrospective cohort study in a tertiary emergency department with 450,000 annual visits. Patients aged ≥18 years presenting with UGIB and undergoing endoscopy between June 1, 2021, and July 31, 2022, were included. Glasgow-Blatchford, pre-endoscopic Rockall, AIMS65, and modified age blood test comorbidity (ABC) scores were calculated using clinical and laboratory data.
Results: Of 795 patients, 142 required endoscopic interventions and 653 did not. The Glasgow-Blatchford score predicted intervention needs (p = 0.007) but lacked clinical relevance. All scores predicted mortality (p < 0.001); however, AIMS65 and the modified ABC score had the highest predictive accuracy, especially in those requiring interventions (p < 0.001).
Conclusion: Pre-endoscopy scores have limited utility in predicting intervention needs. Male gender, hematemesis, melena, elevated urea, and low red cell distribution width (RDW) may indicate intervention requirements. The modified ABC score demonstrated strong predictive value for mortality, making it a clinically relevant tool for UGIB risk stratification.

Background: The potential association between dietary inflammatory index (DII) and colorectal cancer (CRC) risk, as well as colorectal adenomas (CRA) risk, has been extensively studied, but the findings remain inconclusive. We conducted this systematic review and dose-response meta-analysis to investigate the relationship between the DII and CRC and CRA.
Methods: We comprehensively searched the PubMed, Embase, Cochrane Library, and Web of Science databases for cohort and case-control studies reporting the relationship between DII and CRA, or between DII and CRC, as of 15 July 2025.

Rotator cuff injuries are common and procedures of repair have evolved from open techniques to minimally invasive and arthroscopic ones. Despite these advances, the biomechanics, biology, and value of transosseous repairs remain superior, leading to the development of innovative devices that enable the utilization of this technique without the use of anchors, improving both the efficiency and safety of the procedure. This article reviews the latest advances in transosseous rotator cuff repair, highlighting its biomechanical advantages, as well as the factors that enhance recovery and offer more consistent long-term outcomes.

Background And Purpose: Colonoscopy is an essential diagnostic and therapeutic tool for lower gastrointestinal bleeding (LGIB), with colonoscopic hemostasis needed in a subset of patients. We aim to identify risk factors associated with colonoscopic hemostasis requirement in acute LGIB patients.
Methods: This retrospective study examined consecutive patients who underwent colonoscopy for acute LGIB at a single tertiary hospital between November 2020 and May 2023.
