The substantial and growing prevalence of heart failure, which remains the leading cause of preventable hospitalisation worldwide, has brought heart failure prevention into sharp focus. Although this condition has historically been characterised by impaired cardiac function, mounting evidence has underscored its complex and multisystem pathobiology. Epidemiological studies have indicated that other forms of cardiovascular disease, along with kidney and metabolic dysfunction, frequently and increasingly contribute to heart failure onset.

Cardiovascular-kidney-metabolic (CKM) syndrome is a newly defined health syndrome, encompassing the overlapping pathophysiology and risk factors of cardiovascular and kidney disease, obesity, and type 2 diabetes. It is the leading cause of morbidity and mortality worldwide, with an annual health cost of approximately AUD $24 billion in Australia alone. A disproportionate burden is observed among those with adverse social determinants of health.

Hypertension is the greatest risk factor for cardiovascular diseases and significantly contributes to cardiovascular mortality. In Japan, the hypertensive population is estimated at 43 million, with only 27% achieving well-controlled blood pressure (BP). The onset of major cardiovascular events, including stroke, myocardial infarction, heart failure, and aortic dissection, frequently occurs during the early morning hours.

Type 2 diabetes and chronic kidney disease (CKD) are preventable and treatable. Their silent and progressive clinical course calls for structured assessment with timely feedback to patients and care providers for activating decision-making. Apart from CKD, patients with diabetes can have complications affecting multiple organs, notably the cardiovascular system, eyes, and feet.

Purpose Of Review: Overview the current landscape of pharmacoequity in cardiovascular-kidney-metabolic (CKM) syndrome.
Recent Findings: CKM syndrome is a key driver of the significant morbidity and mortality associated with cardiovascular disease, and poses a significant threat to public health. Despite a growing armamentarium of evidence-based therapies for the prevention and management of CKM syndrome, access to these treatments remains unequal.
