Article Abstract

Case: A 79-year-old man with a left total hip arthroplasty presented with left hip pain following a fall. Radiographs and CT scan demonstrated a displaced, T-type, periprosthetic acetabular fracture involving the anterior and posterior columns and disruption of the ischiopubic segment. The inferior pubic ramus and the posterior column fractures were stabilized with percutaneous internal fixation using retrograde fully-threaded 6.5-mm cannulated screws. The patient demonstrated full recovery, obviating a revision hip arthroplasty and permitting early weight-bearing.

Conclusion: Retrograde percutaneous inferior pubic ramus screw fixation is an effective adjunct to posterior column fixation for periprosthetic acetabular fractures, preventing further displacement, facilitating early weight-bearing, and avoiding revision arthroplasty.

Percutaneous pelvic screwing (PPS) enables fixation of traumatic or atraumatic fractures with little or no displacement, or displaced but reduced fractures, and preventive fixation of primary or secondary tumoral lesions. It is a relatively recent technique, and indications are evolving with progress in pre- and intra-operative imaging. Morbidity is lower than with open surgery.

Introduction: The success rate of antegrade insertion of a full-length ramus osseous fixation pathway (OFP) screw remains unreported. The objective of this study was to assess the safety, feasibility, and effectiveness of a novel antegrade technique for inserting full-length ramus screws, as well as to determine the parameters of the ramus OFP based on screw placement.

Patients And Methods: From January 2022 to September 2024, patients with fractures of the superior pubic ramus or the anterior acetabular column treated with a novel technique of an antegrade insertion of a superior ramus OFP screw were recruited into this study.

Objective: To develop a protocol for locating the Xi point using three-dimensional (3D) cone-beam computed tomography (CBCT) and to evaluate the linear distance between the Xi point and the mandibular foramen on the CBCT image.

Study Design: A retrospective cross-sectional study.

Study Setting: The study was conducted on standardised CBCT records of 35 patients aged >18 years (13 men, 22 women).

Alloplastic temporomandibular joint replacement (TMJR) implants have become an acceptable clinical solution for end-stage temporomandibular joint (TMJ) pathology. However, early screw fixation loosening can be a concern. Acoustic emission (AE) has been reported to successfully detect the early deformation of hip implants.
