Article Abstract

Background: Underdiagnosis of peripheral arterial disease results in inadequate treatment and more serious consequences. Hence, clinicians have focused on early diagnosis and treatment.

Aim: To investigate the effectiveness of the combination of doppler ultrasonography (DUS), three-dimensional dynamic contrast-enhanced magnetic resonance angiography (CE-MRA), and CT angiography (CTA) in assessing lower extremity arterial disease in diabetes mellitus (DM).

Methods: This study retrospectively analyzed the imaging and clinical data of 116 patients diagnosed with DM complicated with lower extremity vascular diseases from January 2021 to June 2023. All patients underwent unilateral or bilateral DUS, CTA, and CE-MRA as well as invasive digital subtraction angiography (DSA). The application values of DUS, CE-MRA, and CTA were compared.

Results: A total of 152 lower extremity arteries in the 116 patients were graded following the classification of vascular branches. The Kappa values between DUS and DSA were 0.780, 0.755, and 0.806 for diagnosing moderate stenosis and 0.484, 0.699, and 0.449 for severe stenosis of grade 1 arteries, grade 2 arteries, and grade 3 arteries, respectively. The Kappa values between CE-MRA and DSA were 0.784, 0.814, and 0.835 for diagnosing moderate stenosis and 0.694, 0.748, and 0.606 for severe stenosis of grade 1 arteries, grade 2 arteries, and grade 3 arteries, respectively. The Kappa values between CTA and DSA were 0.900, 0.858, and 0.878 for diagnosing moderate stenosis and 0.882, 0.823, and 0.756 for severe stenosis of grade 1 arteries, grade 2 arteries, and grade 3 arteries, respectively.

Conclusion: DUS, CE-MRA, and CTA demonstrated comparable accuracy in diagnosing lower extremity arterial disease in DM, and the consistency between CTA and DSA diagnoses was higher than the other two imaging methods.

Background: Enucleation has the advantages of preserving function and avoiding pancreaticoduodenectomy for benign and low-grade malignant neoplasms in the pancreatic head. However, laparoscopic enucleation (LEn) of pancreatic head tumors remains challenging in terms of bleeding control and duct integrity preservation because of the complicated blood supply to the pancreatic head and the adjacent relationships of lesions with the main pancreatic duct (MPD), especially for deep-seated or broad-based lesions. Aim        To compare the results of primary percutaneous coronary intervention (PCI) for non-ST-segment elevation acute coronary syndrome (NSTE-ACS) in patients who recently recovered from COVID-19 with those not previously infected with SARS-CoV-2; to establish prognostic criteria for PCI complications, including stent thrombosis and restenosis (ST and SR) and progression of ischemic heart disease, and to determine ways to prevent them.Material and methods         In 2021, middle-aged patients admitted to the Baku Central Clinical Hospital with a diagnosis of acute coronary syndrome who underwent urgent myocardial revascularization using percutaneous balloon angioplasty of the occluded coronary artery (CA) with implantation of a second-generation intracoronary drug-eluting stent were divided into two observation groups: the main group of 123 patients who had COVID-19 in the previous 6 months, and the control group of 112 patients who were not previously infected with SARS-CoV-2. The immediate results of PCI were assessed according to the TIMI scale; complications were assessed both clinically, by the incidence of severe complications (major adverse cardiovascular events, MACE), and angiographically, by the incidence of early and late ST and SR, and de novo stenosis that developed during the two-year observation period.

Background: Low-grade endometrial stromal sarcoma (LG-ESS) is a rare malignant tumor of the female reproductive system with atypical clinical symptoms and slow progression.

Case: A 44-year-old female with a history of intermittent severe dysmenorrhea, previous laparoscopic myomectomy, and uterine artery embolization (UAE) presented with rapidly enlarging pelvic masses. Imaging revealed uterine masses suggestive of leiomyomas, although an adnexal origin could not be excluded.
