Pineal cysts (PCs) are common findings on (Magnetic resonance Imaging) MRI, often incidental in females and asymptomatic throughout life. Rare complications, like pineal apoplexy with acute hydrocephalus, require differential diagnosis and urgent intervention. We report a 19-year-old male with a progressive headache and visual decline. MRI showed a 2.3 cm pineal cyst with hemorrhagic features (apoplexy) causing hydrocephalus. He underwent endoscopic third ventriculostomy and microsurgical resection, both successful. Postoperatively, symptoms resolved completely. Though often benign, complicated PCs can be life-threatening. This case highlights their management, aiding understanding of etiologies, differential diagnoses, and treatments, enhancing medical knowledge.

Papillary tumors of the pineal region (PTPR) are rare central nervous system neoplasms, with a limited number of pediatric cases reported in the literature. Their optimal management remains unclear due to their unpredictable biological behavior and high recurrence rates. This study presents the clinical course, treatment, and long-term follow-up of a 3-year-old child diagnosed with PTPR.

Melatonin induces endoplasmic reticulum (ER) stress, modulates autophagy, and promotes apoptosis in various tumors, including gastric, ovarian, cervical, oral tongue, colorectal, renal, hepatic, and bladder cancer. In placental choriocarcinoma, melatonin reduces cell viability and induces apoptosis by inhibiting autophagy and disrupting the mitochondrial membrane potential.

To describe a case of radiation retinopathy following craniospinal radiation and provide an overview of studies on radiation retinopathy following whole-brain radiotherapy. A retrospective chart review was conducted to collect a single patient's data on demographics, radiation therapy, ophthalmologic examination findings, treatment, and outcomes. For the systematic review, OVID Medline, Embase, and Cochrane CENTRAL were searched for studies reporting on radiation retinopathy following whole-brain radiotherapy.

Background: With recent advancements in neuroendoscopic surgery enabling smaller openings, the role of preoperative evaluation has become increasingly important. Endoscopic surgery has demonstrated particular efficacy in the occipital transtentorial approach (OTA) for pineal region tumors. This study evaluated anatomical factors influencing endoscopic OTA performance and established criteria for optimal side selection.
