Nocardiosis remains a rare and often underdiagnosed bacterial infection, particularly in immunocompromised individuals. The case report by Zhang highlights the diagnostic and therapeutic challenges in managing skin infection in a 93-year-old patient with chronic renal insufficiency. This editorial explores the importance of timely diagnosis, microbiological confirmation, and tailored antibiotic therapy.

Treatment of actinomycotic mycetoma with joint involvement is challenging. We present a patient in India with actinomycotic mycetoma who reached complete cure and remission after linezolid and meropenem treatment with a 2-year posttreatment follow-up. Clinicians should use novel drug regimens based on subspecies variations of Nocardia and regional drug susceptibility patterns to guide therapy.

Department of Burns and Plastic Surgery, Zhongshan City People's Hospital, Zhongshan 528400, Guangdong Province, China.

This letter discusses the critical yet underrecognized intersection of chronic renal insufficiency and skin infection in the case reported by Zhang , emphasizing the diagnostic challenges and therapeutic complexities in the context of advanced age, comorbidities, and immunocompromised populations. The study's strengths included its integration of immunological profiling and precision medicine, demonstrating that a tailored low-dose trimethoprim-sulfamethoxazole regimen with pharmacokinetic monitoring can improve outcomes in geriatric patients with chronic renal insufficiency while mitigating nephrotoxicity risks. However, its limitations included a single-case design, reliance on phenotypic diagnostics, and the lack of information regarding comorbidity interactions.

Cerebral nocardiosis caused by represents a rare and diagnostically challenging infectious disease, predominantly affecting immunocompromised patients. This opportunistic infection may also pose life-threatening risks to immunocompetent individuals. The diagnostic process is frequently complicated by the absence of distinctive clinical manifestations and technical limitations inherent to conventional microbiological detection methods, which collectively impede the acquisition of definitive pathogenic evidence, thereby resulting in diagnostic delays.

Department of Bacteriology and Immunology, Beijing Tuberculosis and Thoracic Tumor Research Institute/Beijing Chest Hospital, Capital Medical University, Beijing, People's Republic of China.

Unlabelled: Drug resistance in (Mtb), especially isoniazid (INH) resistance, challenges tuberculosis control. This study evaluated droplet digital PCR (ddPCR) against the traditional MeltPro TB assay. A total of 77 INH-resistant samples from Beijing Chest Hospital, China, underwent ddPCR, drug susceptibility testing, and Sanger sequencing.
