Introduction The global rise in antimicrobial resistance (AMR) poses a serious public health threat, reducing the effectiveness of commonly used antibiotics against prevalent bacterial infections. The mushrooming of Indian e-pharmacies, especially during and after the COVID-19 pandemic, has improved public access to medicines, including antibiotics. However, the ease of availability, combined with an ambiguous regulatory framework governing e-pharmacies, may inadvertently encourage the irrational use of antibiotics.

Left atrial (LA) decompression is often performed in patients on venoarterial extracorporeal membrane oxygenation to help offload the left ventricle. Atrial septal stents may be used to ensure the adequacy of LA decompression; however, if there is cardiopulmonary recovery and extracorporeal membrane oxygenation support is no longer needed, the stents require removal. We describe 3 pediatric patients who required venoarterial extracorporeal membrane oxygenation support and atrial septal stent placement who underwent successful transcatheter removal of the stents after cardiac recovery.

Importance: Externally controlled trials (ECTs) can serve as an alternative in settings where randomized clinical trials (RCTs) are unfeasible. However, the methodological rigor of ECTs, particularly with regard to bias control, is often inadequately assessed, which can compromise the validity of studies and lead to incorrect decisions.
Objective: To examine the design, conduct, and analysis characteristics of current ECTs and to assess whether appropriate methods were used to control bias.

Background: Radical antegrade modular pancreatosplenectomy (RAMPS) enables posterior dissection along Gerota's fascia, contributing to improved R0 resection rates, lymphadenectomy, and overall survival (OS) in pancreatic body and tail cancers. Although a posterior margin ≥ 1000 μm has been associated with better prognosis, its relative impact compared with Gerota's fascia resection remains unclear. This study evaluated whether securing a posterior margin ≥ 1000 μm improves outcomes in patients undergoing Gerota's fascia resection for pancreatic body and tail cancer.
