Unroofed coronary sinus syndrome is a very rare entity that accounts for only ∼1% of atrial septal defects. Here, we present a case series of patients with unroofed coronary sinus syndrome and discuss its clinical presentation, imaging diagnostics, and management options.

Atrioventricular septal defect (AVSD) with cor triatriatum sinister (CTS) is a rare congenital anomalous combination. Few cases of AVSD and CTS have been reported in the literature. However, an isolated case of CTS with partial AVSD, left superior vena cava, and unroofed coronary sinus in an adult has not been reported in the literature.

Hypertrophic cardiomyopathy (HCM) is an inherited disease present in 1 in 500 individuals and is the most common cause of sudden cardiac death in children. We present the case of a 17-year-old boy with HCM and a primary prevention subcutaneous implantable cardioverter-defibrillator (S-ICD) who developed left ventricular outflow tract obstruction and a myocardial bridge of the left anterior descending coronary artery. The patient underwent a septal myectomy/myotomy and muscular bridge unroofing.

Background: A 17-year-old male athlete experienced out-of-hospital cardiac arrest due to ventricular fibrillation during exercise. Myocardial bridging (MB) and vasospasm were identified as potential contributors to the event.
Case Summary: Initial imaging revealed MB of the mid-left anterior descending artery embedded within hypertrophic myocardium.
