Article Abstract

Background: Platypnea-orthodeoxia syndrome (POS) is a rare condition; however, the number of reported cases has been increasing in recent years.

Case Summary: We present a case of POS caused by a patent foramen ovale (PFO), where oxygen saturation and right atrial pressure were measured in both the supine and the sitting positions before and after PFO closure. After closure, oxygen desaturation during postural changes resolved, but the increase in right atrial pressure persisted.

Discussion: Reports of right atrial pressure measurement in POS are rare. To the best of our knowledge, this is the first case in which right atrial pressure was measured both before and after PFO closure.

Take-home Messages: Changes in right atrial pressure, in addition to postural blood flow alterations, contribute to the pathophysiology of POS in certain cases. Understanding individual case variations is crucial for effective management.

Objective: Large hemispheric infarction (LHI) represents one of the most severe subtypes of ischemic stroke, associated with high rates of disability and mortality. Aims: The PARACHUTE-HF trial (NCT04023227) is evaluating the effect of sacubitril/valsartan compared with enalapril on a hierarchical composite of cardiovascular events (cardiovascular death, first heart failure hospitalization), and change in N-terminal pro-B-type natriuretic peptide (NT-proBNP) levels in participants with heart failure and reduced ejection fraction (HFrEF) caused by chronic Chagas cardiomyopathy (CCC). We describe the baseline characteristics of participants in PARACHUTE-HF compared with prior HFrEF trials.

Methods And Results: PARACHUTE-HF, a multicentre, active-controlled, open-label trial, enrolled 922 participants with confirmed CCC, New York Heart Association (NYHA) functional class II-IV, and left ventricular ejection fraction (LVEF) ≤40%.
