Article Abstract

Endoscopic resection techniques have been extensively applied in the treatment of precancerous lesions and early-stage cancers in the digestive tract. Both endoscopic submucosal dissection (ESD) and most endoscopic mucosal resection (EMR) procedures rely heavily on submucosal injection-a critical step that ensures complete lesion removal and helps prevent postoperative bleeding and perforation. This technique has also greatly contributed to the advancement and widespread adoption of endoscopic procedures. The selection of appropriate submucosal injection materials merits attention. This paper expounds on the current research status of submucosal injection materials and envisions the future development directions of submucosal injection materials.

Rationale: Prolapsed hemorrhoids can impair quality of life due to associated symptoms such as pain. While hemorrhoidectomy is considered the gold standard for treating prolapsed hemorrhoids, this procedure inevitably involves complications such as postoperative pain, bleeding, and delayed recovery. Therefore, there is an increasing need for treatment options that are immediate, effective, and minimally invasive, while also taking into account patients' physical and social backgrounds, preferences, and values.

Conventional gelatin's gel-to-sol transition upon heating restricts its utility in biomedical applications that benefit from a gel state at physiological temperatures such as Pluronic F127 and poly(NIPAAm). Herein, we present "rev-Gelatin", a gelatin engineered with reverse thermo-responsive properties that undergoes a sol-to-gel transition as temperature rises from ambient to body temperature. Inspired by the phase dynamics of common materials like candy and ice cubes, whose surfaces soften or partially melt under warming, facilitating inter-object adhesion- rev-Gelatin leverages this concept to achieve fluidity at room temperature for easy injectability.

Unusual Bridge-Shaped Hyperplastic Polyp in the Sigmoid Colon.

CRSLS

September 2025

Faculty of Medicine, Department of Internal Medicine, Division of Intensive Care Medicine, Çanakkale Onsekiz Mart University, Çanakkale, Türkiye. (Dr. Demirer Aydemir).

We present a rare morphological variant of a colonic polyp observed during a routine screening colonoscopy. A 62-year-old male with known chronic obstructive pulmonary disease (COPD) and benign prostatic hyperplasia (BPH) was found to have a bridge-shaped polyp in the sigmoid colon. The polyp was successfully resected via snare polypectomy following submucosal adrenaline injection.

Hereditary angioedema (HAE) is a rare, potentially life-threatening disorder characterized by recurrent, disabling episodes of subcutaneous or submucosal swelling. Lanadelumab, a monoclonal antibody targeting plasma kallikrein, is approved for long-term prophylaxis and has shown high efficacy in clinical trials. However, real-world data on its prolonged use, particularly from East Asia, remain scarce.

Electrolyte depletion syndrome (EDS), also known as McKittrick-Wheelock syndrome, is a rare but life-threatening condition caused by secretory diarrhea from colorectal villous tumors, often accompanied by severe electrolyte imbalances and renal dysfunction. Large, circumferential tumors have traditionally been managed with surgical resection, frequently requiring stoma formation. Recently, endoscopic submucosal dissection (ESD) has emerged as a minimally invasive alternative, although its feasibility for large rectal tumors remains limited.
