Article Abstract

A 53-year-old woman with hyperinsulinemic hypoglycemia was diagnosed with multiple insulinomas as determined by contrast-enhanced CT. Genetic test confirmed the presence of multiple endocrine neoplasia type 1 (MEN1). 68Ga-exendin-4 PET/CT revealed multiple lesions with intense uptake in the tail and uncinate process of the pancreas, with an additional lesion in the gastric wall on the lesser curvature. These lesions were subsequently confirmed to be insulinomas by histopathologic examination after surgery. This case illustrates the benefits of 68Ga-exendin-4 PET/CT in detecting multiple and ectopic insulinomas in patients with a MEN1 background.

A 52-year-old man was pathologically diagnosed with a pancreatic neuroendocrine tumor (pNET) and synchronous liver metastasis through debulking surgery 2 years ago. Recently, the patient showed recurrent severe hypoglycemia after receiving multiple treatments. 68Ga-Exendin-4 PET/CT imaging revealed diffuse uptake in the skeleton and other metastatic sites, which 68Ga-DOTANOC PET/CT grossly underestimated.

Purpose: Accurate preoperative localization is imperative for the treatment of insulinomas. Glucagon-like peptide-1 receptor (GLP-1R) imaging has demonstrated remarkable efficacy in localization of insulinomas. The aim of this study was to assess the diagnostic performance of GLP-1R PET/CT with Ga-exendin-4 in localization of insulinoma, and to provide evidence for clinical practice from a real-world study.

Article Synopsis
  • - A 35-year-old woman experienced recurrent hypoglycemia, leading to a diagnosis of insulinoma after several imaging tests failed to locate the tumor.
  • - A specialized imaging technique, Ga-Exendin-4 PET/CT scan, was used during her travels and successfully identified a tumor in the pancreas, confirmed by surgery.
  • - Following the removal of the tumor, the patient's blood sugar levels normalized, demonstrating the effectiveness of the imaging method and the need for access to advanced diagnostic facilities.
