Although the pathogenesis of seropositive rheumatoid arthritis (RA) remains unclear, studies suggest that pulmonary inflammation-related biological mechanisms play a significant role in its development. This review thoroughly explores the mechanisms underlying early pulmonary lesions in seropositive RA, focusing on the mucosal barrier hypothesis, neutrophil extracellular traps, pathogenic microbial infections like COVID-19, Vitamin D, the microbiome and gut-lung axis, inhalation exposures and chronic pulmonary diseases. This study seeks to provide novel insights and theoretical foundations for the prevention and treatment of seropositive rheumatoid arthritis.

Objectives: To systematically review and meta-analyse the risk factors proposed by the American College of Rheumatology and American College of Chest Physicians as screening tools for rheumatoid arthritis-associated interstitial lung disease (RA-ILD), focusing exclusively on studies using high-resolution computed tomography (HRCT) in prospectively collected data from unselected RA patients.
Method: A comprehensive search was conducted to identify studies evaluating RA-ILD risk factors. Selection criteria included studies using HRCT in prospective, unselected RA cohorts.

Background/aims: To identify factors associated with achieving low disease activity (LDA) after 48 weeks of targeted therapy in patients with rheumatoid arthritis (RA) despite not meeting treat-to-target (T2T) criteria at week 24.
Methods: Data were collected from a multicenter, prospective observational cohort of Korea patients with RA receiving targeted therapy between April 2020 and July 2023. Patients who continued their initial targeted therapy despite not achieving LDA at week 24 were assigned to the LDA and non-LDA groups at week 48.

Transverse myelitis (TM) is an inflammatory disorder of the spinal cord often associated with autoimmune diseases, such as systemic lupus erythematosus (SLE) or neuromyelitis optica spectrum disorder (NMOSD); however, it is rarely linked to rheumatoid arthritis (RA). We present the case of a 28-year-old woman with subacute ascending numbness, lancinating pain, and bilateral lower extremity weakness resulting in significant functional impairment. Despite upper motor neuron signs on examination and supportive cerebrospinal fluid findings, including elevated gamma globulins and positive myelin basic protein, spinal MRI remained negative.

Caplan syndrome, a rare condition characterized by the coexistence of pneumoconiosis and rheumatoid arthritis in individuals exposed to inorganic dust, remains a relevant diagnostic consideration in the 21st century. We present a case of a 41-year-old former coal miner who developed multiple cavitary pulmonary nodules years after leaving the mining sector, initially mimicking pulmonary malignancy. Extensive diagnostic work-up, including positron emission tomography-computed tomography, bronchoscopy and lung biopsy, eventually led to the diagnosis of Caplan syndrome, with seropositive rheumatoid arthritis emerging 8 years after exposure cessation.
