Article Abstract

Background: Single chamber atrial pacing (AAI) provides a disease-specific treatment for sick sinus syndrome (SSS) but has largely been replaced by DDD pacing. With the advent of leadless atrial pacemakers (PM), there is growing interest in long-term follow-up data in patients with SSS and an AAI pacemaker.

Purpose: To assess the incidence of system upgrade in patients treated with AAI-PM for SSS during long-term follow-up.

Methods: This is an analysis of prospectively enrolled patients undergoing implantation of an AAI-PM. Wenckebach block point (WBP) was measured at implantation and serially during follow up.

Results: We included 178 patients (58% female, median age at implantation 77 [71-83] years). The median follow-up duration was 6.5 [2.0-9.7] years. Twenty-three patients (13%) received a system upgrade to a DDD system, corresponding to a yearly upgrade rate of 2.0%. Median time to system upgrade was 5.2 [1.6-8.7] years. Reasons for system upgrade were higher-degree AVB (39%), atrial arrhythmias (35%), low WBP (17%), and syncope (9%). Median WBP at implantation was 130 [120-140] bpm, showing a significant decline over time in the upgrade-group compared to the rest of the cohort with 103 [91-130] bpm vs. 130 [120-130] bpm (p = 0.011).

Conclusion: In this cohort of patients undergoing AAI-PM implantation for SSS, upgrade to a DDD system was low during long-term follow-up. Therefore, AAI pacing for the treatment of SSS may be considered a patient-tailored treatment option, especially in light of novel leadless pacing therapies.

Nano-laminography combines the penetrating power of hard X-rays with a tilted rotational geometry to deliver high-resolution, three-dimensional images of laterally extended, flat specimens that are otherwise incompatible with, or difficult to image using, conventional nano-tomography. In this work, we demonstrate a full-field, X-ray nano-laminography system implemented with the transmission X-ray microscope at beamline 32-ID of the upgraded Advanced Photon Source at Argonne National Laboratory, USA. By rotating the sample around an axis inclined by 20° to the incident beam, the technique minimizes the long optical path lengths that would otherwise generate excessive artifacts when planar samples are imaged edge-on.

During the vaccine production through the chick embryo cultivation method, harmful cracks may occur from the perforation of a trocar on the eggshell, around the impact hole, leading to the failure of cultivation. To detect the perforative cracks, this study proposes a method based on acoustic responses. By stimulating the embryo eggs and collecting the acoustic signals, 7 characteristic values were extracted from the time and the frequency domains: The maximum value in the time domain; The difference in the time domain; The frequency-domain peaks, 870 Hz, 1250 Hz, 1470 Hz and 1770 Hz; The mean value of the waveform.

A century of observations: the Ertel meridian circle at Stockholm Observatory.

In the 1820s, Stockholm Observatory undertook a significant upgrade of its instruments. The new acquisitions included a meridian circle, crafted by Traugott Ertel in Munich, which - after extensive delays - was finally installed at the observatory in 1834. Despite its advanced capabilities, the instrument saw limited use during its first forty years, as observatory directors were largely occupied with geodetic projects for the Crown.

Controlling vehicular emissions is a critical priority, particularly in developing countries like Brazil, where the vehicular fleet has expanded significantly. Although Brazil's Program to Control Vehicular Emissions has reduced certain air pollutants by mandating technological advancements in new vehicles, it did not consider the substantial increase in vehicle numbers and density across the country.
