Fungal keratitis is a severe corneal infection most commonly caused by filamentous fungi. Even with prompt treatment with current antifungals, it often results in corneal perforation and blindness. In this report, we observe that the beta-adrenergic antagonist, propranolol, displays antifungal activity against and corneal isolates o and strikingly blocks disease establishment in a murine model of keratitis. These findings suggest that beta-blockers have potential as a novel FK treatment.This study is registered with ClinicalTrials.gov as NCT00997035 (MUTT Trial).

Objective: To determine the effect of adjunctive rose-bengal photodynamic therapy (RB-PDT) in the treatment of fungal, Acanthamoeba, and smear/culture negative infectious keratitis.
Study Design: This international, randomized, double-masked, sham controlled clinical trial, randomizes patients with corneal ulcers in a 1:1 fashion to one of two treatment arms: 1) Topical antimicrobial plus sham RB-PDT or 2) Topical antimicrobial plus RB-PDT.
Main Outcome Measures: The primary outcome was best spectacle-corrected visual acuity (BSCVA) at 6 months.

Extracellular vesicles (EVs) derived from corneal epithelial cells have shown great promise in promoting corneal wound healing and stromal regeneration, but they face challenges with rapid clearance from the eye. This study addresses these challenges by developing a biocompatible collagen-hydrogel sustained delivery system. We successfully isolated, purified, and characterized corneal epithelial EVs (CE-EVs), assessed their efficacy in corneal epithelial healing in vitro, and demonstrated their sustained delivery over 10 days followed by an on-demand release through enzymatic degradation of the hydrogel, which mimics the in vivo scenario.

Our group developed a novel dual-chamber corneal storage vial (DCV) comprised of two compartments that become isolated by the corneal button and prevent epithelial and endothelial interaction. We hypothesize that the DCV results in similar levels of corneal swelling and corneal endothelial cell (EC) death compared to the standard vial (SCV) and is safe for corneal storage. To assess corneal thickness and EC density, human cornea pairs (N = 18) were recovered by the Florida Lions Eye Bank, with one cornea from each pair stored in the SCV and the other in the DCV.
