Complications related to the coronary sinus following cardiac surgery are rare and require a high index of suspicion to ensure timely diagnosis and treatment. We describe the clinical presentation and management of a patient with atrioventricular septal defect who developed coronary sinus ostial obstruction following left atrioventricular valve replacement. This manifested as a decline in ventricular function and supraventricular arrhythmias. Surgical enlargement of the coronary sinus ostium was initially successful but failed to produce a durable result.Meeting presentation: Oral presentation at the Congenital Heart Surgeons' Society Annual Meeting; October 19, 2023; Boston, MA.

Effective therapy for patients suffering from refractory angina remains a major unmet need. Chronic angina, which is refractory to medical and interventional therapies, affects patients who are not suitable for revascularization, patients following successful revascularization, and patients with coronary microvascular dysfunction. Coronary sinus (CS) narrowing has been studied as a potential therapy for patients with angina.

Background: We evaluated the surgical outcomes of modified right ventricle (RV) overhaul (mRVOh), implemented as part of comprehensive management for pulmonary atresia with intact ventricular septum (PA-IVS).
Methods And Results: Twenty-five mRVOh procedures were performed in 23 patients with PA-IVS without RV-dependent coronary circulation. The procedure involved RV sinus myectomy, infundibular muscle resection, and tricuspid valve (TV) and pulmonary valve (PV) repair.

Introduction And Importance: Cardiac atherosclerotic disease (CAD), the leading cause to cardiovascular disease-related death, coexisting with cardiac myxoma, the commonest benign primary cardiac neoplasm, is rare. The treatment of the comorbidity is complicated due to the rarity and inexperience.
Case Presentation: A 65-year old male patient with intermittent chest tightness and shortness of breath for 10 years was admitted for further work-up.
