Article Abstract

Inherited retinal diseases (IRDs) are a group of rare Mendelian disorders that often result in progressive vision loss and potentially to complete blindness at the end stage. In this study, we investigated a large cohort of patients with IRDs from Pakistan, the world's fifth most populous country, which is also characterized by distinctive demographic features, such as a high prevalence of consanguinity, endogamy, and a wide variety of ethnic groups. Specifically, we examined a total of 213 unrelated families (722 affected individuals) from three very large geographical regions. We achieved precise molecular diagnosis in 171 pedigrees (80.3%) and detected causative variants in 60 different IRD-associated genes, revealing a mutational landscape that differed substantially from previous data from other European or Asian populations, heavily shaped by endogamy and rare or recurrent founder mutational events. To our knowledge, this work represents the largest genetic study on IRDs within the Pakistani population.

Methods: Twenty-one HVs and three IRD patients were imaged. Macular variation in 20 HVs in CC metrics (CC density, CC diameter, CC tortuosity, void diameter, void area, lobule count, lobule area, and RPE-CC distance) were assessed by imaging a 28° strip of overlapping AO-OCTA volumes (3° × 3°) from the optic nerve head to the temporal macula.

Fragile X syndrome (FXS), a leading inherited cause of intellectual disability and autism, is frequently accompanied by sleep and circadian rhythm disturbances. In this study, we comprehensively characterized these disruptions and evaluated the therapeutic potential of a circadian-based intervention in the fragile X mental retardation 1 () knockout (KO) mouse. The KO mice exhibited fragmented sleep, impaired locomotor rhythmicity, and attenuated behavioral responses to light, linked to an abnormal retinal innervation and reduction of light-evoked neuronal activation in the suprachiasmatic nucleus.

Personalised genomic strategies improve diagnostic yield in inherited retinal dystrophies: a stepwise, patient-centred approach.

Background: Inherited retinal dystrophies (IRDs) are a genetically heterogeneous group of conditions, with approximately 40% of cases remaining unresolved after initial genetic testing. This study aimed to assess the impact of a personalised genomic approach integrating whole-exome sequencing (WES) reanalysis, whole-genome sequencing (WGS), customised gene panels and functional assays to improve diagnostic yield in unresolved cases.

Subjects/methods: We retrospectively reviewed a cohort of 597 individuals with IRDs, including 525 probands and 72 affected relatives.

Objectives: To provide insights into the financial burden and opportunity cost of vision loss from retinitis pigmentosa (RP) in the US by using net present value (NPV) of direct medical and nonmedical costs.

Methods: Assumptions, including economic (discount rate, median income, cost-of-living, Social Security and Medicare taxes, public insurance/supplemental benefits, nutrition assistance, and prescription drug assistance), medical (federal National Health Expenditure tables, a recent retrospective claims analysis, and Optum Health claims database) and demographic (mortality rate, increase in mortality due to visual impairment, progression of blindness, probability of survival, retirement rate, rate of disability, and RP diagnosis probability) were made to develop a NPV model. Scenario analyses were performed on benefits and costs arising from patients with RP, if vision could be preserved via novel gene therapies.
