Objective: This study investigated the value of multiparametric MRI (mpMRI) in predicting Gleason score (GS) upgrading and downgrading in radical prostatectomy (RP) compared with presurgical biopsy.
Materials And Methods: Clinical and mpMRI data were retrospectively collected from 219 patients with prostate disease between January 2015 and December 2021. All patients underwent systematic prostate biopsy followed by RP. MpMRI included conventional diffusion-weighted and dynamic contrast-enhanced imaging. Multivariable logistic regression analysis was performed to analyze the factors associated with GS upgrading and downgrading after RP. Receiver operating characteristic curve analysis was used to estimate the area under the curve (AUC) to indicate the performance of the multivariable logistic regression models in predicting GS upgrade and downgrade after RP.
Results: The GS after RP was upgraded, downgraded, and unchanged in 92, 43, and 84 patients, respectively. The AUCs of the clinical (percentage of positive biopsy cores [PBCs], time from biopsy to RP) and mpMRI models (prostate cancer [PCa] location, Prostate Imaging Reporting and Data System [PI-RADS] v2.1 score) for predicting GS upgrading after RP were 0.714 and 0.749, respectively. The AUC of the combined diagnostic model (age, percentage of PBCs, tPSA, PCa location, and PI-RADS v2.1 score) was 0.816, which was larger than that of the clinical factors alone ( < 0.001). The AUCs of the clinical (age, percentage of PBCs, ratio of free/total PSA [F/T]) and mpMRI models (PCa diameter, PCa location, and PI-RADS v2.1 score) for predicting GS downgrading after RP were 0.749 and 0.835, respectively. The AUC of the combined diagnostic model (age, percentage of PBCs, F/T, PCa diameter, PCa location, and PI-RADS v2.1 score) was 0.883, which was larger than that of the clinical factors alone ( < 0.001).
Conclusion: Combining clinical factors and mpMRI findings can predict GS upgrade and downgrade after RP more accurately than using clinical factors alone.

Unlabelled: REMS-BMD by radiofrequency echographic multispectrometry is primarily determined by a patient's BMI, age, and sex. Only about 2.8% of the changes in femoral neck REMS-BMD can be attributed to replacement of the total hip with metal implants.

Introduction: International medical oncology societies recommended early integration of palliative care to alleviate symptoms, improve quality of life and significantly reduce aggressiveness of care near the end of life, defined as the use of chemotherapy, high rate of hospitalisation/visits to emergency or intensive care unit in the last month of life or death in acute care unit. However, the most appropriate schedule for patient referral is still to be determined. Scores and criteria are debated, with uncontrolled symptoms (pain, dyspnoea, distress, etc) being one of the main indicators.

Objective: To measure discrepancies in risk adjustment scores using only Medicaid or Veterans Health Administration (VA) diagnoses for Veterans dually enrolled in VA and Medicaid.
Study Setting And Design: Veterans aged 18-64 enrolled in the VA and Medicaid for at least one full calendar year during 2017-2020. We compared the number and overlap of annual diagnoses derived from VA and Medicaid data.

Background: Orphanage children often experience dental anxiety due to limited exposure and a lack of support. This research explores the therapeutic potential of music-based interventions like musical drama and music listening as alternatives to the traditional Tell-Show-Do (TSD) technique to alleviate anxiety.
Aim: To compare the effectiveness of musical drama and music listening over the TSD technique in reducing dental anxiety among 12-15-year-old orphanage children.
Testosterone modulated by exercise plays a pivotal role in maintaining the overall health of both males and females. Therefore, this study aimed to determine the effects of an integrated exercise approach on total testosterone levels during different phases of the menstrual cycle in eumenorrheic females. This was a two-armed parallel design, single-blinded, randomized controlled trial held from March 14, 2023, to February 21, 2024, in Aadil Hospital Defense Lahore.
