Objective: To wean respiratory support, preterm infants with severe respiratory failure are often administered systemic corticosteroids. We sought to evaluate if postnatal age or clinical characteristics predicted death or tracheostomy following systemic dexamethasone in evolving bronchopulmonary dysplasia.
Study Design: We performed a retrospective study of infants born at ≤ 30 weeks' gestational age cared for at a Level IV referral center from 2009 to 2019 who received a complete course of systemic dexamethasone beyond 4 weeks of age for the indication of preventing death and/or liberating from positive pressure ventilation.

Rationale: We report an extremely rare case in which delayed diagnosis and treatment of Mycobacterium tuberculosis infection primarily involving the subcutaneous tissues of an extremity led to hematogenous dissemination of the infection and subsequent deterioration of the patient.
Patient Concerns: An 82-year-old man presented to our hospital with a painful mass on the right ankle for over a year, as well as persistent fever and shortness of breath for >14 days. He received piperacillin/tazobactam followed by meropenem, which failed to decrease his peak temperature.

Using high-flow nasal cannula (HFNC) in patients with hypoxemic respiratory failure to avoid intubation raises concerns about its potential to increase mortality due to delayed intubation. Identifying at-risk patients is essential. While the literature predicts risk with oxygen-based indices (ROX, SpO2/FiO2, PaO2/FiO2), we aimed to detect ventilation insufficiency.
