Behaviorally informed "nudges" are widely used in government outreach but are often seen as too modest to address poverty at scale. In four field experiments over 2 y ( = 542,804 low-income households), we test whether more proactive communication, varying message framing, and more precise targeting can boost take-up of tax-based benefits in California above and beyond traditional light-touch approaches. Our interventions focused on extremely vulnerable households, most with no prior-year earnings, who were at risk of missing out on two crucial benefits: the 2021 expanded Child Tax Credit and pandemic-relief Economic Impact Payments.

Background: Lateral malleolar avulsion fracture (LMAF) and subfibular ossicle (SFO) are distinct entities that both present as small bone fragments near the lateral malleolus on imaging, yet require different treatment strategies. Clinical and radiological differentiation is challenging, which can impede timely and precise management. On imaging, magnetic resonance imaging (MRI) is the diagnostic gold standard for differentiating LMAF from SFO, whereas radiological differentiation on computed tomography (CT) alone is challenging in routine practice.

We report a 64-year-old woman who developed symptomatic vasospasm on postoperative day 7 after clipping of an unruptured right middle cerebral artery (MCA) aneurysm. Imaging revealed right MCA vasospasm, which resolved with oral antiplatelets and intravenous vasodilators. She was discharged without neurological deficits.
