An 80-year-old woman presented with the sudden onset of vertigo and vomiting and was diagnosed with acute cerebellar infarction. Twelve days after the onset, she experienced positional vomiting, predominantly in the lying-down position, triggered by postural changes with immediate or delayed onset. Over 1 month, 23 episodes of vomiting occurred. Brain computed tomography revealed edema in the infarcted area, fourth ventricular outlet compression, and lateral ventricular enlargement. These findings suggest that fluctuations in intracranial pressure are associated with intermittent obstructive hydrocephalus exacerbated by the patient's posture. This case highlights the importance of considering postural triggers in patients with recurrent vomiting associated with cerebellar infarctions.

Renal cysts are common, typically asymptomatic, fluid-filled sacs that rarely require intervention. Nevertheless, in rare cases, large symptomatic cysts can cause significant morbidity. We report the case of an 87-year-old man presenting to the emergency department with right chest wall pain following a ground-level fall, accompanied by worsening nausea, vomiting, and decreased oral intake over 6-7 months.

Situs inversus totalis (SIT) is a rare congenital anomaly involving mirror-image reversal of thoracic and abdominal organs, which may complicate the diagnosis and management of acute abdominal conditions due to atypical symptom localization. This report describes a 23-year-old male who presented with lower abdominal pain and vomiting. Clinical examination revealed tenderness in the left iliac fossa and laboratory findings showed leukocytosis.

Background: Situs inversus totalis is an uncommon congenital condition characterized by the complete reversal of internal organ placements from their usual orientation. This syndrome presents challenges in both diagnostic and surgical care owing to the anatomical reversal, with an incidence rate of around 1 in 5000 to 20,000 infants.
Case Presentation: A 50-year-old Asian Sindhi Ayan female patient with well-managed hypertension presented with intermittent cramp-like discomfort in the upper left region of her abdomen, which exacerbated after consuming fatty foods, accompanied by feelings of nausea and vomiting.

Background And Aims: Ultrasonographic (USG) optic nerve sheath diameter (ONSD) provides a real-time, non-invasive method for assessing intracranial pressure. This study investigates perioperative ONSD variations and contributing factors in patients undergoing elective intracranial tumour resection.
Methods: A prospective observational study was conducted on 94 adults with intracranial tumours, excluding orbital lesions and sellar/suprasellar tumours.
