: The objective assessment of visual function is crucial in glaucoma management, highlighting the importance of electroretinography (ERG). This study investigates the diagnostic performance of photopic negative response (PhNR) amplitude and the normalized PhNR/b-wave ratio in diagnosing glaucoma, focusing on the impact of myopia. : Ninety-one glaucoma patients and 19 glaucoma suspects were included, defining myopia as axial length (AL) > 24 mm or > 25 mm. Full-field photopic ERG used a red stimulus on a blue background. : Myopic glaucoma patients showed a higher PhNR/b-wave ratio than non-myopic patients ( = 0.023). AL negatively correlated with b-wave amplitude (r = -0.239, = 0.012). PhNR amplitude demonstrated an area under the receiver operating characteristic curve (AUC) of 0.661 ( = 0.028) overall and was less effective in the myopic subgroup (AUC = 0.574, = 0.082). The diagnostic performance of the PhNR/b-wave ratio did not achieve statistical significance in either the total group (AUC = 0.616, = 0.114) or the myopic subgroup (AUC = 0.574, = 0.332). : Standardization using the PhNR/b-wave ratio did not enhance diagnostic accuracy over PhNR amplitude, particularly in myopic patients, underscoring the need for careful interpretation in myopia and further research to optimize electrophysiological diagnostics.

Purpose: To investigate the correlation between the photopic negative response (PhNR) of the light-adapted flash electroretinography (ERG) and measurements of standard automated perimetry (SAP) and optical coherence tomography (OCT) in assessment of retinal ganglion cells' (RGCs) affection in glaucoma.
Patients And Methods: A cross-sectional study included 40 eyes of glaucoma patients and 40 eyes of age- and gender-matched normal subjects. Participants underwent a complete ophthalmologic assessment, SAP, OCT, and light-adapted flash ERG using the extended PhNR protocol of the International Society for Clinical Electrophysiology of Vision (ISCEV).

The photopic negative response of the electroretinogram reflects retinal ganglion cell function and consequently aids diagnosis of optic nerve diseases including glaucoma. In this study, we assessed the efficacy of stimulation parameters for electroretinographic recordings of the multifocal photopic negative response (mfPhNR) for the detection of glaucoma and compared the diagnostic accuracy of electrophysiological, structural and functional measures of glaucoma. We compared the diagnostic performance of the mfPhNR for 6 different stimulation rates in a cohort of 24 controls, 10 glaucoma suspects (GLA ) and 16 glaucoma participants (GLA).

Importance: Retinal ganglion cells endure significant metabolic stress in glaucoma but maintain capacity to recover function. Nicotinamide, a precursor of NAD , is low in serum of glaucoma patients and its supplementation provides robust protection of retinal ganglion cells in preclinical models. However, the potential of nicotinamide in human glaucoma is unknown.
