Sunken upper eyelid is a morphological symptom that surrounds the eye with aging; however, its causes remain poorly understood. We investigated the correlations between the sunken upper eyelid and morphological parameters inside the eye area using magnetic resonance imaging (MRI), verified the relationship between age and degree of sunken upper eyelid photographically, and quantified muscle fibers histologically classified by myosin heavy chain (MYH) species and connective tissues in the superior palpebral part of the orbicularis oculi muscle from cadaveric specimens of various ages. The results showed the development of sunken upper eyelids, decreased thickness and number of MYH4-positive muscle fibers, and increased collagen and elastic fiber proportions in the orbicularis oculi muscle with age. On MRI, both the thickness of the orbicularis oculi muscle and the vertical position of the globe within the orbit were negatively correlated with the sunken upper eyelid. Changes in the orbicularis oculi muscle offer new insights into the mechanisms underlying age-related morphological changes in the periocular area.

Background: The subbrow approach is reportedly more effective than upper blepharoplasty for improving lateral hooding. Additionally, it allows for the removal of thick periorbital fat while simultaneously tightening weakened and sagging orbicularis oculi muscles. This study introduces a method for repositioning and redistributing the medial fat pad to the central area, restoring fullness in the central area, and improving medial upper eyelid bulging.

Objective: Through intraoperative observation, histological analysis of specimens, and animal experiments on fat transplantation, we initially explore the histological characteristics of fat from different parts and provide theoretical support for related clinical operations.
Methods: The discarded orbital fat and subcutaneous fat tissue were collected from 18 patients who underwent upper eyelid and abdominal surgery at the same time, and 9 of them were stained with HE, Masson, and immunofluorescence to observe the histological characteristics of orbital fat and abdominal fat. The other 9 cases were transplanted to both sides of the back of 6-week-old nude mice.

Background: Correcting sunken eyelids is challenging. This is because the eyelid skin easily moves according to orbital fat volume, making it difficult to predict, which can lead to asymmetrical or unfavorable results. Currently, the preoperative design method for understanding the Rouleau phenomenon and using it to push the lower bag can be helpful as it reduces asymmetry and offers greater predictability.
