Purpose: Myocardial infarction results in extensive cardiac remodeling that can lead to heart failure. Human pluripotent stem cell-derived cardiomyocyte (hPSC-CM) injection can improve heart function but may lead to engraftment-associated ventricular tachycardia (VT). Optogenetics uses light stimulation to control electrical activity of cells genetically modified to express light-sensitive proteins (opsins).

Feasibility of Machine Learned Intracardiac Electrograms to Predict Postinfarction Ventricular Scar Topography.

Circ Arrhythm Electrophysiol

July 2025

Department of Cardiology, Westmead Hospital, Sydney, New South Wales, Australia (K.D.S., T.G.C., R.G.B., S.T., A.B., D.S., Y.K., C.-J.H., J.J.H.C., E.K., S.K.).

Background: Accurate delineation of scar patterns is valuable for guiding catheter ablation of ventricular tachycardia. We hypothesized that scar and its pattern of distribution can be determined from intracardiac electrograms using computational signal processing and that further improvements in classification can be achieved with a convolutional neural network.

Methods: A total of 5 sheep underwent anteroseptal infarction (plus 1 healthy control) with electroanatomic mapping (129±12 days post-infarct).

Effect of leadless left ventricular endocardial and left bundle branch area pacing on biventricular repolarization metrics.

Heart Rhythm

May 2025

Department of Biomedical Engineering and Imaging Sciences, King's College London, London, United Kingdom; Department of Cardiovascular Medicine, Guy's and St Thomas's NHS Foundation Trust, London, United Kingdom.

Background: Cardiac resynchronization therapy (CRT) delivered with left ventricular (LV) epicardial pacing may increase arrhythmic risk through detrimental effects on ventricular repolarization. Leadless LV endocardial CRT including leadless left bundle branch area pacing (LBBAP) may mitigate this by preserving a more physiological transmural activation pattern.

Objective: This study aimed to evaluate the effect of leadless LV endocardial and leadless LBBAP on repolarization metrics derived from electrocardiographic imaging (ECGi).

Recent advances in three-dimensional (3D) ultra-high-density mapping systems have uncovered previously unknown mechanisms underlying various arrhythmias. This clinical review, titled "3D Mapping Quest," focuses on the recently uncovered mechanisms of four types of arrhythmias. (1) To elucidate the precise mechanisms underlying atrial fibrillation (AF), ultra-high-density mapping with adequate spatial and temporal resolution is essential.
