Parent Artery Occlusion (PAO) is a valid treatment choice in giant internal carotid artery (ICA) aneurysms of the cavernous segment when the preoperative balloon test occlusion (BTO) demonstrates an adequate cross circulation from the contralateral side. A high flow arterial bypass is, however, mandatory if the result suggests otherwise or is indeterminate. We present here a case of a 72-year lady where the BTO results were inconclusive. Rather than proceeding for the ICA ligation and a high flow bypass straight away, we replicated the BTO in the operation theatre, using intraoperative electrophysiologic neuromonitoring under the general anaesthesia. A temporary clip was applied on the ICA followed by gradual reduction of the blood pressure up to 30% from the baseline and sustained for 30 min. The motor evoked potential (MEP) and somatosensory evoked potential (SSEP) monitoring did not show ipsilateral hemispheric ischemia. Assured by this electrophysiologic evidence, we proceeded with ligation of the cervical ICA. The patient recovered from surgery without any neurological deficits and remains functionally intact at 4-years follow-up. Based on this case, this novel method can be considered a further refinement of the conventional BTO, allowing clarification of the inconclusive results that may sometimes be encountered.

Objectives: This study aimed to analyse the number of myocardial infarction (MI) admissions during the COVID-19 lockdown periods of 2020 and 2021 (March 15th to June 15th) and compare them with corresponding pre-pandemic period in 2019. The study also evaluated changes in critical treatment intervals: onset to door (O2D), door to balloon (D2B) and door to needle (D2N) and assessed 30-day clinical outcomes. This study examined MI care trends in India during the COVID-19 lockdown period, irrespective of patients' COVID-19 infection status.

Fungal cerebral aneurysms, particularly those resulting from direct invasion by fungal sinusitis, are rare and often fatal when involving the cavernous segment of the internal carotid artery (ICA). We present a case of a ruptured fungal ICA aneurysm caused by sinusitis, successfully treated with parent artery occlusion (PAO). In this case, an 80-year-old woman presented with right ptosis, facial pain, and cranial nerve III, IV, and VI palsies.

This study aimed to compare the high-resolution anorectal manometry (HRAM) profiles of patients with CC and healthy volunteers (HV), as well as patients with functional defecation disorders (FDD) and those without in the constipation group.
Methods: This retrospective comparative study included patients with CC who underwent HRAM and a balloon expulsion test (BET).

Purpose: This study aims to evaluate the safety and efficacy of the CGuard dual-layer stent with its mesh embolic protection system (EPS) in elective cases for treatment of internal carotid artery stenosis and compares it to the Carotid Wallstent as benchmark.
Methods: In this retrospective, multicenter study, we analyzed data from consecutive patients who underwent carotid artery stenting with CGuard at two high-volume neurointerventional centers and compared them with prior consecutive patients treated with Carotid Wallstent (CWS), with and without a balloon guiding catheter (BGC) as protection, at the same institutions. Patient demographics, procedural details, clinical complications, early in-stent thrombosis and occlusion rates, and late follow-up restenosis rates were assessed.
