Hepatopulmonary syndrome (HPS), defined by the presence of pulmonary vascular dilatations that cause right-to-left transpulmonary shunting of venous blood with a consequential increase in the alveolar-arterial oxygen gradient, is a relatively frequent complication of chronic liver disease. While orthotopic liver transplantation (OLT) is indicated and often curative in HPS patients with end-stage liver disease (ESLD), little is known about the peri- and post-operative-period risks of CVA in OLT recipients with HPS. : We report a case series of five non-consecutive OLT recipients with HPS who developed ischemic and/or hemorrhagic CVAs during or shortly after OLT, raising concern that the risks of neurological complications remain increased even after OLT. Our case series hopes to highlight the importance of close vigilance in this subset of patients, at a time when there may be multiple issues to be addressed in facilitating post-operative recovery.

Combining two advanced therapies may improve outcomes in Crohn's disease (CD) refractory to monotherapy. We conducted a descriptive case series of 27 patients with CD who initiated combination therapy with upadacitinib and infliximab (n = 1), risankizumab (n = 17), ustekinumab (n = 3) or vedolizumab (n = 6). At 12 weeks, 24 achieved clinical response and 9 achieved steroid-free remission.

Purpose: To report pyramidal-like, hyperreflective changes of the outer retina and retinal pigmented epithelium (RPE) in three patients with an atypical non-syphilitic outer retinopathy.
Study Design/materials And Methods: Single institutional case series conducted at the National Eye Institute, National Institutes of Health.
Results: Hyperreflective, pyramidal lesions of the outer retina and RPE have been described in patients with syphilitic posterior segment uveitis.

Jockeys endure considerable physical and psychological demands, rendering them vulnerable to occupational injuries such as fractures, concussions, and soft tissue damage. This descriptive case series presents the medical and occupational histories of three professional jockeys with long-term disabilities following work-related accidents. Each individual commenced their jockey career in early adolescence.
