Bacterial meningitis and infectious cavernous sinus thrombosis (CST) are both life-threatening central nervous system infections, often caused by sinusitis. While cerebrovascular complications are well-recognized in bacterial meningitis, their association with CST is rare. A 69-year-old man presented with a 19-day history of headache, followed by diplopia.

Purpose: To report a surgical treatment for neovascular angle closure glaucoma.
Observations: A 69 year-old man with proliferative diabetic retinopathy developed neovascular angle closure with intraocular pressure (IOP) 60 mm Hg. Surgical goniosynechialysis and placement of a Hydrus canalicular stent were combined with pupilloplasty using an iris cerclage suture to maintain tension on the peripheral iris to limit reformation of goniosynechiae.

Downhill varices, a rare manifestation of venous hypertension, arise from superior vena cava (SVC) obstruction. We present a unique case of downhill varices secondary to SVC syndrome in a 69-year-old man on chronic total parenteral nutrition who presented with large volume melena and facial swelling. In this case, chronic central venous catheter use contributed to thrombotic occlusion of the SVC, leading to collateral vessel formation and variceal development.

Good syndrome (GS), alternatively termed thymoma with immunodeficiency, is a rare adult-onset immunodeficiency disorder characterized by concurrent thymoma and hypogammaglobulinemia, accompanied by defects in both B-cell-mediated immunity and T-cell-mediated immunity. Owing to the non-specific clinical presentation, diagnosis is frequently delayed, resulting in poor prognosis and elevated mortality. In this study, we report the case of a 69-year-old man with GS who presented with symptoms of recurrent cough and productive sputum.

Background: Immune checkpoint inhibitors (ICIs) have revolutionized cancer treatment but are associated with various adverse effects, including myocarditis, with mortality rates up to 50%. We report a case of fatal ICI-related fulminant eosinophilic myo-pericarditis complicated by tamponade in a 69-year-old man with metastatic lung adenocarcinoma.
Case Summary: Two weeks after receiving one dose of pembrolizumab, the patient presented with sudden chest pain and dyspnoea.
