Eloquent brain creates a challenge when resecting brain arteriovenous malformations (bAVMs). Here the authors present their technique of using subcortical motor mapping as an adjunct to increase safety during resection of a high-grade bAVM involving somatosensory cortex as well as cortical spinal tracts and visual tracts. After a bilateral craniotomy, they use direct cortical stimulation of the left motor cortex and subcortical stimulation using a suction stimulator to dynamically map motor tracts during the resection. They get within 3 mm of the corticospinal tracts by stimulation. The video can be found here: https://stream.cadmore.media/r10.3171/2024.10.FOCVID24107.

Background: The objective of this study is to investigate the predictive role of O6-methylguanine-DNA methyltransferase (MGMT) and isocitrate dehydrogenase (IDH) status on the efficacy of bevacizumab (BEV) in high-grade glioma (HGG), while excluding the interference of chemotherapy agents.
Methods: A retrospective, single-center analysis was conducted on 103 patients with HGG who received BEV treatment. The enrolled patients were grouped based on their different biomarker statuses.

Background: High-grade astrocytoma with piloid features (HGAP) was recently added to the WHO 2021 CNS classification system among the group of circumscribed astrocytic gliomas. These tumors present with high-grade piloid histology with similarities to glioblastoma. HGAPs in the pineal region become particularly challenging due to its deep location and proximity to deep venous structures, the midbrain, and the thalamus.

Adult fibrosarcoma is a rare and aggressive soft tissue malignancy originating from spindle-shaped fibroblasts, primarily affecting deep soft tissues in the extremities, trunk, head, and neck. Surgical resection with R0 margins remains the gold standard, with adjuvant radiation therapy recommended for large or high-grade tumors to reduce recurrence risk. Chemotherapy and immunotherapy play complementary roles in disease management, with emerging strategies targeting matrix metalloproteinases and tumor microenvironment interactions to enhance chemosensitivity and overcome drug resistance.
