Background: We present a multimodality imaging study of a rare case of postsurgical chronically evolved pseudoaneurysm with a possible rupture buffered by the huge thrombus.

Case Summary: A patient known for previous late presentation myocardial infarction complicated by shock and ventricular septal defect and treated with surgical repair and triple coronary artery bypass grafting, was directed to our hospital for severe mitral regurgitation. Computed tomography, cardiac magnetic resonance, and echocardiography, in a multimodality approach, revealed a huge postsurgical cardiac pseudoaneurysm, with an extensive thrombus and the native pericardium not perfectly distinguishable from pseudoaneurysm tissue or surgical patch.

Purpose: Postsurgical pain (PSP) is a common complication in surgical patients that can progress to chronic pain and opioid dependence. Current analgesics, including opioids and non-opioid agents, are limited by short durations of action and adverse effects. This study reports the development and evaluation of extended-release bupivacaine microparticles (BuMPs) designed to provide sustained local analgesia and improve post-surgical pain management.

However, global research implementation varies significantly due to patient demographics, healthcare infrastructure, and economic factors, creating disparities in access that directly impact patient care and public health outcomes.

Objective: To evaluate the global landscape of SCS research by analyzing patient demographics, geographic distribution, funding sources, and pain conditions treated across chronic pain populations aged 18 years and older, with primary focus on associations between geographic regions and patient age demographics.

Exocrine pancreatic insufficiency (EPI) arises from a deficiency in pancreatic enzymes, leading to malabsorption and varied symptoms such as gastrointestinal discomfort, weight loss and steatorrhea. Though EPI commonly follows chronic pancreatitis or pancreatic surgery, its delayed onset is rarely discussed. We report a unique case of EPI manifesting 29 years after pancreaticoduodenectomy.
