Acute transverse myelitis (ATM) is a disease characterized by inflammation of the spinal cord and may have various causes. In the context of this work, the distinction between isolated ATM and initial manifestation of autoimmune-mediated diseases of the central nervous system such as multiple sclerosis (MS) is crucial. Hence, the aim of this work was to identify predictive factors associated with the conversion to definite MS in a collective of individuals after their initial episode of isolated ATM (no initial identified cause). In this retrospective data analysis from the Vienna MS Database, all patients from Jan. 1, 1999, to Dec. 31, 2019, with a diagnosis of isolated ATM (according to the criteria of the Transverse Myelitis Consortium Working Group) who underwent lumbar puncture were extracted. Electronic medical records were reviewed on the availability of clinical data including therapy and follow-up, laboratory results including cerebrospinal fluid (CSF) analysis, evoked potentials (EP) as well as magnetic resonance imaging data. Among 42 patients with the diagnosis of isolated ATM, 12 (29%) were subsequently diagnosed with MS over a median follow-up period of 7.7 years. Univariately, MS converters were younger (32 years [25-39] vs. 42 years [31-50], p = 0.032), had a lower CSF/serum albumin ratio (29 [24-35] vs 37 [27-52], p = 0.037), lower CSF total protein (4.5 [2.8-4.8] vs. 5.5 [3.4-8.5], p = 0.023) and a higher proportion of CSF-specific oligoclonal bands (OCB; 83% vs. 30%, p = 0.002). In the multivariate regression analysis, the presence of CSF-specific OCB emerged as the sole predictive factor of subsequent MS diagnosis (OR: 14.42, 95% CI 1.39 to 149.48, p = 0.03). In a collective of 42 patients with isolated ATM and an MS conversion rate of nearly 30%, the only but highly predictive factor were CSF-specific OCB. This emphasizes the significance of conducting timely CSF analysis in such patients and underscores the need for tailored monitoring and follow-up strategies in this specific group.

Oxidation of the Chatt-type tungsten dinitrogen compound, -(depe)W-(N) (depe = EtPCHCHPEt), with [(η-CH)Fe]-[BAr ] (BAr = B-(3,5-(CF)CH)) resulted in isolation of [(depe)WN]-[BAr ], a rare example of a tungsten-(IV) nitride prepared from N cleavage. A bimetallic μ-N ditungsten intermediate supported by terminal N ligands was identified, and irradiation with visible light promoted dinitrogen cleavage and formation of [(depe)WN]-[BAr ]. Performing the analogous one-electron oxidation of the related tungsten dinitrogen compound, -(dppe)W-(N) (dppe = PhPCHCHPPh), furnished the corresponding cationic, 17-electron tungsten dinitrogen complex, [(dppe)W-(N)]-[BAr ], that was characterized by X-ray diffraction and vibrational and EPR spectroscopies.

Background: Extensively drug-resistant (XDR) gram-negative pathogens represent a critical therapeutic challenge in intensive care units, with mortality rates exceeding 50 %. The synergistic combination of ceftazidime-avibactam with aztreonam offers a novel therapeutic approach, particularly in carbapenemase-producing Enterobacterales.
Methods: This prospective observational study analysed 24 critically ill adult ICU patients with confirmed XDR gram-negative infections from October 2024 to April 2025.
Cefepime-taniborbactam (FEP-TAN) and meropenem-xeruborbactam (MEM-XER) are β-lactam-β-lactamase inhibitor (BL-BLI) combinations currently in development and both projected to treat metallo-β-lactamase (MBL)-producing Gram-negative pathogens. Among Gram-negative pathogens, the low permeability of the outer membrane of poses unique challenges to drug discovery in general and to BL-BLIs in particular. This study set out to augment β-lactam antibiotic potency by enhancing outer membrane permeability of using novel amphiphilic aminoglycoside-based outer membrane permeabilizers.

This study introduces the synthesis and detailed characterization of six novel nonsymmetric (E/Z)-isomeric mixtures of 1,3-diether-2-alkenes derived from precursors with a glycerol skeleton: 1-((3-butoxyallyl)oxy)heptane ([H, A, B]), 1-(2-methoxyethoxy)-5-methylhex-2-ene ([ME, A, IP]), 1-((3-(2-methoxyethoxy)prop-1-en-1-yl)oxy)butane ([ME, A, B]), ((3-isopropoxyprop-1-en-1-yl)oxy)benzene ([IP, A, Ph]), ((3-(2,2,2-trifluoroethoxy)prop-1-en-1-yl)oxy)benzene ([F, A, Ph]), and 1-(allyloxy)-3-(2,2,2-trifluoroethoxy)prop-1-ene ([F, A, A]). These compounds were synthesized via a three-step reaction starting from various glycidyl ethers, purified by fractional distillation and drying, and obtained with overall isolated yields of at least 60%. The ratio of isomer configurations (E:E:Z:Z) varied based on substituents R and R.
