Article Abstract

Retinal surgery is a challenging procedure requiring precise manipulation of the fragile retinal tissue, often at the scale of tens-of-micrometers. Its difficulty has motivated the development of robotic assistance platforms to enable precise motion, and more recently, novel sensors such as microscope integrated optical coherence tomography (OCT) for RGB-D view of the surgical workspace. The combination of these devices opens new possibilities for robotic automation of tasks such as subretinal injection (SI), a procedure that involves precise needle insertion into the retina for targeted drug delivery. Motivated by this opportunity, we develop a framework for autonomous needle navigation during SI. We develop a system which enables the surgeon to specify waypoint goals in the microscope and OCT views, and the system autonomously navigates the needle to the desired subretinal space in . Our system integrates OCT and microscope images with convolutional neural networks (CNNs) to automatically segment the surgical tool and retinal tissue boundaries, and model predictive control that generates optimal trajectories that respect kinematic constraints to ensure patient safety. We validate our system by demonstrating 30 successful SI trials on pig eyes. Preliminary comparisons to a human operator in robot-assisted mode highlight the enhanced safety and performance of our system.

Purpose: The purpose of this study was to assess the reflectivity of the outer retinal layers (ORLs) in patients with central serous chorioretinopathy (CSCR) and to examine the relationship between the dimensions of the subretinal fluid (SRF) and ORL.

Methods: This retrospective, cross-sectional study included 33 eyes of 33 patients with CSCR and 33 age- and gender-matched controls. Unnormalized and relative reflectivities for the retinal pigment epithelium (RPE), the external limiting membrane (ELM), and the ellipsoid zone (EZ), as well as SRF height, base width, and area, were measured on optical coherence tomography images.

The Von Hippel-Lindau disease (VHL) is an autosomal dominant condition characterized by multiple cystic tumors in several organs, including the pancreas. The symptoms are variable, and suspicion must be raised with typical lesions, such as a hemangioblastoma of the central nervous system (CNS) or retina, associated with a renal cell carcinoma, a pheochromocytoma or multiple pancreatic cysts, besides neuroendocrine tumors (NET). The diagnosis in a patient without a family history should be suspected in case of a hemangioblastoma of the CNS and/or retina, which could also be associated with other lesions, such as pancreatic cysts and NETs.

Introduction: The purpose of this study was to evaluate the effects of three brief interventions (spoken/guided meditation, nature sounds, or music) on patients' subjective experience with intravitreal injections (IVIs).

Methods: A total of 121 consecutive patients were randomized into four cohorts: spoken/guided meditation (n = 31), nature sounds (n = 30), music (classical or jazz, n = 30), and control (n = 30). Subjective anxiety levels were recorded prior to and following a three- to five-minute intervention in the first three study groups as well as following the IVI.

Introduction: Autoimmune uveitis is a sight-threatening inflammatory eye disease driven by immune dysregulation. We previously introduced a therapeutic strategy involving the induction of retinal-antigen-specific regulatory T cells (Tregs) via αCD4 antibody injection followed by administration of the retinal self-peptide IRBP1-20, which effectively suppresses inflammation during the onset of experimental autoimmune uveitis (EAU).

Methods: We evaluated the long-term therapeutic efficacy of this approach in a chronic EAU model.
