Bilateral orbital metastases are extremely rare, and no previous literature has reported bilateral multiple orbital metastases treated with stereotactic radiosurgery (SRS). We present the impressive images of a patient with history of metastatic leiomyosarcoma with newly diagnosed bilateral multiple orbital metastases. The pre-SRS magnetic resonance imaging (MRI) showed 1 right and 2 left orbital metastatic lesions. The patient underwent SRS, and the post-SRS MRI at 2 months showed a decrease in size of the treated lesions. The positron emission tomography scan at 5 months after the treatment showed no avid F-fluorodeoxyglucose uptakes in the 3 orbital metastases. The presenting symptoms were all improved after the treatment with no adverse effects from the SRS. Our interesting images show the presence of multiple lesions in the orbital spaces with excellent treatment response post SRS. This is the first report of a patient who has bilateral and multiple orbital metastases that were successfully treated with standalone SRS.

Background And Purpose: The choroid of the eye is a rare site for metastatic tumor spread, and as small lesions on the periphery of brain MRI studies, these choroidal metastases are often missed. To improve their detection, we aimed to use artificial intelligence to distinguish between brain MRI scans containing normal orbits and choroidal metastases.
Materials And Methods: We present a novel hierarchical deep learning framework for sequential cropping and classification on brain MRI images to detect choroidal metastases.

Solitary fibrous tumor, formerly also referred to as hemangiopericytoma, is a fibroblastic mesenchymal neoplasm of the orbit that rarely metastasizes but has a propensity for local recurrence. We describe 5 cases of orbital solitary fibrous tumors that were treated with radiation. In 3 patients, radiation therapy was given in the neoadjuvant setting prior to surgery.

We present a rare case of delayed retrobulbar and adrenal metastases from renal cell carcinoma (RCC), diagnosed 5.5 years after radical nephrectomy. The patient exhibited symptomatic orbital involvement, with imaging revealing a hypervascular retrobulbar mass and an incidental right adrenal lesion, indicative of an oligometastatic state.

Primary mucinous adenocarcinoma of the orbit is an exceptionally rare epithelial malignancy that closely mimics metastatic disease both clinically and radiologically, often delaying definitive diagnosis. We report the case of a 69-year-old man who presented with progressive ocular pain, axial proptosis, and a well-circumscribed orbital mass on imaging. Histopathological evaluation following incisional biopsy revealed a mucinous adenocarcinoma with signet ring morphology.
