We introduce a discrete mathematical model for the mechanical behaviour of a planar slice of human corneal tissue, in equilibrium under the action of physiological intraocular pressure (IOP). The model considers a regular (two-dimensional) network of structural elements mimicking a discrete number of parallel collagen lamellae connected by proteoglycan-based chemical bonds (crosslinks). Since the thickness of each collagen lamella is small compared to the overall corneal thickness, we upscale the discrete force balance into a continuum system of partial differential equations and deduce the corresponding macroscopic stress tensor and strain energy function for the micro-structured corneal tissue. We demonstrate that, for physiological values of the IOP, the predictions of the discrete model converge to those of the continuum model. We use the continuum model to simulate the progression of the degenerative disease known as keratoconus, characterized by a localized bulging of the corneal shell. We assign a spatial distribution of damage (i.e. reduction of the stiffness) to the mechanical properties of the structural elements and predict the resulting macroscopic shape of the cornea, showing that a large reduction in the element stiffness results in substantial corneal thinning and a significant increase in the curvature of both the anterior and posterior surfaces.

Purpose: To investigate the mechanism, intraoperative characteristics, management, and prevention of incision capsular bag herniation (ICBH), a previously unreported complication during cataract surgery in eyes with lens subluxation.
Methods: A retrospective observational case series was conducted on five male patients who developed ICBH during phacoemulsification with intraocular lens (IOL) implantation between January 2019 and December 2024. Among 867 subluxated-lens surgeries performed during this period, the estimated incidence of ICBH was 0.

Purpose This study aims to compare the initial three-month outcomes of a single-center experience with small incision lenticule extraction (SMILE) for correction of myopia and myopic astigmatism using the VisuMax 500 (Carl Zeiss Meditec, Jena, Germany) versus the VisuMax 800 (SMILE Pro®; Carl Zeiss Meditec, Jena, Germany). This experience is compared to the US Food and Drug Administration approval studies and published literature. Patients and methods The initial 45 eyes (23 patients) that underwent SMILE with the VisuMax 500 in 2018 were compared with the initial 42 eyes (21 patients) that underwent SMILE Pro® with the VisuMax 800 in 2024.

Background: Sjögren's syndrome (SS) is a chronic autoimmune disease delineated by excessive lymphocyte infiltration to the lacrimal or salivary glands, leading to dry eye and dry mouth. Exosomes secreted from mesenchymal stem cells (MSC) are known to have anti-inflammatory and tissue regeneration abilities. This study endeavored to demonstrate the effect of MSC-derived exosomes on the clinical parameter of dry eyes and associated pathology in SS mouse model.

Purpose: To present a case of synthetic intrastromal corneal ring segment (ICRS) intrusion secondary to necrosis and migration, managed by implantation of corneal allogenic intrastromal ring segments (CAIRS) within the preexisting tunnel.
Methods: A 24-year-old man with known keratoconus underwent bilateral ICRS implantation. He presented with blurred vision in the right eye 6 weeks after the procedure.
